Underlying Principles
(E.G., DELEGATION,
LEVELS OF PREVENTION,
ADVANCE DIRECTIVES)
HELLP syndrome: is a
variant of GH in which
hematologic conditions
coexist w/severe
preeclampsia involving
hepatic dysfunction.
HELLP syndrome is
diagnosed by laboratory
tests, not clinically.
H: hemolysis resulting in
anemia and jaundice
EL: elevated liver enzymes
resulting in elevated
alanine aminotransferase
or aspartate transaminase,
epigastric pain, and
nausea/vomiting
LP: low platelets resulting
in thrombocytopenia,
abnormal bleeding/clotting
time, bleeding gums,
petechiae, and possibly
disseminated intravascular
coagulopathy
REVIEW MODULE CHAPTER ___________
Nursing Interventions
WHO? WHEN? WHY? HOW?
HELLP: severe continuous
headache
Nausea
Blurring of vision
Flashes of lights or dots
before the eyes
No single profile identifies
risks for gestational
hypertensive disorders, but
some high risks include:
maternal age younger than
19 or older than 40
first pregnancy
extreme obesity
multifetal gestation
chronic renal disease
chronic hypertension
familiar hx of preeclampsia
diabetes mellitus
Rheumatoid arthritis
systemic lupus
erythematosus
Physical Assessment
Findings:
Hypertension
Porteinuria
Periorbital, facial, hand and
abdominal edema
Pitting edema of lower
extremities
Vomiting
Oliguria
Hyperreflexia
Scotoma
Epigastric pain
Right upper quadrant pain
Dyspnea
Diminished breath sounds
Seizures
Jaundice
Manifestations of
progression of
hypertensive disease
w/indications of worsening
liver involvement, kidney
failure, worsening
hypertension, cerebral
involvement, and
developing coagulopathies.
