Pharmacology Scenario 3: Danielle Young Bear Guided Reflection Questions Scenario Analysis Questions PCC Bear? [1] What priority problem(s) did you identify for Danielle Young The priority problem for Danielle Young Bear was her intense lower back pain, which would be considered chronic pain as it was first diagnosed about a year ago. After doing a neurological assessment and taking vitals, not much stood out as severely abnormal. She did have cold, sweaty skin, which may support the fact that she was in pain. Her lung sounds were clear, despite her cold. S/EBP/PCC Provide medication reconciliation for this patient. Danielle Young Bear reported that she has been taking: Cyclobenzaprine 5mg 3x/day, Duloxetine 60mg 1x/day, Tramadol 50mg 46x/day, and Hydrocodone 5mg/Acetaminophen 325mg 4x/day. She also reported taking Nyquil 3 tablespoons/day, Acetaminophen 500mg 4x/day, and 1 cup of Willow Bark tea occasionally. She reported no allergies. I did not administer any medication to this patient. Per the doctor’s orders, I drew blood to test for her liver function. S/EBP/PCC reconciliation? What problems did you identify during the medication She has been taking several medications that contain Acetaminophen, and her total daily intake has been exceeding the 4,000 mg limit per 24hours. Additionally, the combination of Tramadol and Cyclobenzaprine can increase Danielle’s risk of seizures. Her total daily intake of Acetaminophen is calculated as follows, with the last number being from her Nyquil dose: (325*4)+(500*4)+(325*3)= 4275mg Acetaminophen (before the occasional cup of Willow Bark tea) S/EBP/PCC Provide patient education related to information found during the medication reconciliation. Danielle was educated about the risks of excessive use of medication, particularly Acetaminophen, and the risks of liver damage. She was informed that Acetaminophen may be present in OTC medications and herbal supplements, in addition to her prescription medications, so she should consult with a pharmacist before purchasing OTC meds and supplements. Danielle was informed that the total amount of Acetaminophen she takes in 24hours should not exceed 4000mg or 4g. I Explore evidence-based alternative pain management resources to assist with pain control for Danielle Young Bear. Danielle Young Bear may find some relief for her chronic pain through the use of acupuncture, chiropractic therapy, massage, meditation, aromatherapy, and yoga. She is already taking an anti-depressant, which has shown to be effective in some cases of chronic pain. Concluding Questions Describe how you would apply the knowledge and skills that you obtained in Danielle Young Bear’s case to an actual patient care situation. After working through Danielle Young Bear’s case, I would now know to look out for Acetaminophen in OTC cold medicines and herbal supplements (such as Willow Bark). I would also now know to promptly check hepatic function after discovering that a patient has been consuming excessive Acetaminophen.
