Internuclear Ophthalmoplegia When looking to the left side: Left Eye (L): Moves leftwards, controlled by the Lateral Rectus (LR) muscle and the VI Nucleus. Right Eye (R): Moves leftwards, controlled by the Medial Rectus (MR) muscle and the III Edinger-Westphal (EW) Nucleus via the Medial Longitudinal Fasciculus (MLF). Explanation: MLF is responsible for coordinated eye movement when looking to a side. In Multiple Sclerosis (MS), the MLF may get demyelinated, causing the connection to be lost. The Left Eye (L) moves towards the left side, but the Right Eye (R) can't adduct. To compensate, the VI Nucleus of the Left Eye (L) gets hyperactivated, leading to Nystagmus in the normal eye. Effects: Normal Eye (N/E): When looking laterally: Nystagmus (+) Convergence (+) Affected Eye: No eyeball movement Convergence (+) (because only the III cranial nerve is involved on both sides)
