NURSING CARE PLAN Identified Problem: Teenage Mother/Young Maternal Age, Abnormal Family structure Nursing Diagnosis: Risk for impaired parenting CUES OBJECTIVES Short term objectives: Risk factors: ● Patient is only 17 years old ● Father of the baby lives in another city ● Patient is separated from his father since she was 8years old ● After 8 hours of nursing intervention the pt. will be able to enumerate concepts and roles of parenting, and to create realistic goals for her parenting role as well as identify personal strengths, individual needs and methods to achieve these goals. Her mother is not around because of work as an OFW INTERVENTIONS 1. Perform physical assessment w/ the mother present, and demonstrate common newborn characteristics and behaviors. 2. Encourage pt. participation in care behaviors such as putting on diapers, breast feeding, and bathing. 3. Observe the attachment behaviors between the parent and newborn. Recognizing the pt’s family history and experiences may help. 4. Create an environment in which the relationship can be strengthened. Long term objectives: After 3 days of nursing interventions the pt. will be able to demonstrate parenting techniques and assume responsibility for emotional, mental, and physical care and well being of her newborn.Furthermore, pt. will also express confidence in taking care of her baby. 5. Encourage expression of feelings such as frustration, anger, or anxiety, while setting limits on unacceptable behavior. 6. Discuss to the patient about proper parenting roles and teach effective parenting methods 7. Educate patient about proper breastfeeding technique and on ways to properly hold the newborn 8. Provide information about time management, and stress-reduction techniques RATIONALE 1. To promote parent’s knowledge of newborn physical characteristics and behaviors. 2. To promote familiarity with behaviors and decrease parental anxiety and to enhance parental feeling of contribution as newborn’s primary caretakers 3. Identifying factors that may influence the failure to bond between mother and newborn will help us address them beforehand, so that it cannot affect subsequent parent-child interactions EVALUATION Short term: After 8 hours of nursing intervention the short term goals were met. The pt. was able to enumerate concepts and roles of parenting, and was able to create realistic goals for her parenting role as well as identified personal strengths, individual needs and methods to achieve the goals. 4. A safe environment where the parent can freely express their thoughts and feelings can help improve learning and positive interactions between mother and newborn therefore enhancing relationships. 5. The expression of feelings promotes self-understanding and enhances connections with others. Setting limits to unacceptable behaviors can help preserve self-esteem. 6. Promote the patient's knowledge on her role as a parent and enhance understanding towards proper parenting methods. 7. Using the proper breastfeeding technique is important in establishing breastfeeding, to ensure milk transfer and to prevent breastfeeding problems. Using the proper techniques for holding the baby will reduce the risk for accidents and injuries. 8. Discovering ways to reduce stress and anxiety promotes the parent’s ability to deal with problems that may arise during her parenting. Long term: After 3 days of nursing interventions the long term goals were met. The pt. was able to demonstrate parenting techniques and assume responsibility for emotional, mental, and physical care and well being of her newborn. Furthermore, pt. Was also able to express confidence in taking care of her baby.
