Innovations in wearable devices, cloudbased platforms, and analytical algorithms have enabled real-time physiological data collection, remote monitoring, and early detection of health disorders. The implementation of this technology shows promising results, including improved accuracy in measuring vital parameters, effectiveness in detecting critical conditions, and expanded access to health services, especially for people in underserved areas. However, the implementation of IoT-based health monitoring systems faces a number of challenges, such as data security, interoperability, and clinical validation. To optimize its potential, future research needs to focus on strengthening patient data security and privacy. With the support of cutting-edge technologies such as Artificial Intelligence (AI), Machine Learning (ML), and the integration of Electronic Health Records (EHR), IoT-based health monitoring systems have the potential to bring about a major transformation in healthcare services while empowering individuals to take a more active role in managing their health conditions. INTRODUCTION Advances in information and communication technology have opened up new opportunities in various fields. One of the most significant innovations in the last decade is the Internet of Things (IoT), which allows devices to connect and exchange data automatically [1]. Due to its wide application, ranging from the health, agriculture, MICHAEL KURNIAWAN 1 Study Case sports, to security sectors, the number of devices connected to the internet continues to increase [2], [3]. It is estimated that the number of global IoT devices will reach nearly 30 billion by 2030 [4]. In the healthcare sector, IoT presents a great opportunity to improve the quality and efficiency of services through smart and integrated health monitoring systems [5], [6], [7]. These systems enable real-time and continuous monitoring of patient conditions without requiring intensive involvement from medical personnel [8], [9], [10]. IoT technology relies on sensors to collect physiological data, such as heart rate, body temperature, blood pressure, oxygen saturation, and other vital parameters. The data is then sent to a local server or cloud platform for analysis, allowing medical personnel to monitor patients remotely and take medical action quickly [11], [12], [13]. Various studies have proven the effectiveness of IoT implementation in health monitoring. For example, the use of wearable devices such as smartwatches with heart rate and blood oxygen sensors has been proven to have high accuracy [5], [14]. Systems such as WISE (Wearable IoT-cloud-based Health Monitoring System) enable real-time transmission of health data to the cloud, allowing medical personnel to access information at any time [8], [15]. The implementation of IoT systems in rural areas has also been proven to assist in the early diagnosis and treatment of diseases, including COVID-19, with a low rate of vital sign measurement errors [9], [12], [16]. Furthermore, the use of machine learning and deep learning algorithms in health data analysis improves the accuracy and resilience of monitoring systems [6], [17]. However, data security is a major challenge. Studies show that the use of advanced encryption and authentication mechanisms can protect patient data from cyber threats, maintaining the privacy and integrity of health information [18], [19]. In addition, a fog computing approach has been proposed to improve data processing efficiency while reducing the risk of data center failure [20], [19]. In this paper, we discuss the innovations, implementation, and challenges of applying IoT in health monitoring systems based on various recent studies. The aim is to provide a comprehensive overview of technological advances while offering insights for further development in the health sector. MICHAEL KURNIAWAN 2 Study Case RESEARCH METHODOLOGY This study is a literature review that aims to analyze the innovation and implementation of IoT in health monitoring systems [6]. The literature review methodology was conducted systematically to obtain a comprehensive understanding of the basic concepts, latest trends, benefits, challenges, and practical applications of this technology in the context of health services. Identification of Data Sources The main sources were scientific journals, conference proceedings, books, and official documents related to the application of IoT in health monitoring. Data were obtained through online searches using international academic databases, such as PubMed, IEEE Xplore, ScienceDirect, and Google Scholar, with the keywords: “IoT in healthcare monitoring,” “healthcare sensors,” and “wearable devices in healthcare.” Selection Criteria Articles were selected based on the following inclusion criteria: (1) discussing the implementation of IoT in health monitoring, (2) describing innovations in health sensors, (3) explaining the role of cloud platforms in health data analysis, and (4) highlighting challenges and solutions in IoT implementation. Irrelevant or low-quality articles were excluded from the analysis. Data Analysis The selected literature was systematically reviewed to identify key themes, including basic concepts of IoT in healthcare, key components of the system, implementation in hospitals and home monitoring, benefits of use, challenges of implementation, and the latest technological developments. Synthesis of Findings The results of the analysis were synthesized to provide a comprehensive overview of how IoT is used in health monitoring, its benefits, emerging challenges, and the latest technology trends. Literature Review Compilation Based on the findings, a literature review was compiled with an academic structure that included: introduction, literature review, discussion, and conclusion. This section also covered previous research methodologies, algorithms, pseudocode, and procedures used, including evaluation and data collection approaches. Literature Selection Process Using the PRISMA Method Literature selection was carried out using the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) method to ensure that the articles analyzed were relevant and of high quality. Of the 854 articles found, an initial screening was conducted to remove duplicates and inaccessible articles, leaving 704 articles. Next, 200 articles were eliminated because they were not relevant to the topic, leaving 504 articles for further review. At the inclusion stage, 482 articles were removed because they did not meet quality standards, leaving 22 articles selected for in-depth analysis. The selected articles covered important topics, such as the application of IoT for real-time health monitoring with wearable devices, remote monitoring through telemedicine, evaluation of the accuracy of IoT systems in early disease detection, and challenges related to security, privacy, and standardization. This study highlights both the advantages and limitations of IoT in health monitoring, while providing recommendations on best practices and standards needed to support safe and effective implementation in the United States. MICHAEL KURNIAWAN 4 Study Case RESULTS AND DISCUSSION The application of the Internet of Things (IoT) in health monitoring has grown rapidly, with significant potential to improve the quality of healthcare services and public welfare. This literature review highlights various systems and innovations that utilize IoT technology to deliver more effective health monitoring. The implementation of IoT in healthcare services marks a new era, where proactive monitoring, early diagnosis, and timely intervention are increasingly possible. The innovations reviewed include wearable devices capable of collecting physiological data in real-time to remote monitoring systems that allow patients and medical personnel to stay connected without geographical limitations. Studies show that the use of smartwatches with wireless sensor platforms [5] and wearable sensor-based systems [8], [7], [23], [25] can record vital parameters such as heart rate, oxygen saturation, and activity levels, thereby supporting continuous health monitoring. The ability of IoT to transmit health data to cloud-based platforms [8], [9], [11], [12], [10], [13], [26], [19], [23], [27] has revolutionized remote healthcare services, benefiting patients in rural areas, the elderly, and those with chronic diseases. Furthermore, IoT-based analytics enable real-time anomaly detection and early warning alerts [6], [9], [11], [12], [10], [22], [23], [27], allowing for prompt medical intervention. Studies also report high accuracy in measuring vital parameters [9] and effectiveness in detecting critical conditions such as heart attack risk [22]. In fact, this technology has been used to monitor the health of soldiers on the battlefield [28]. However, a number of challenges still need to be overcome. Key issues include patient data security, the need for stronger encryption, authentication, and the use of blockchain [18], [19]. In addition, the lack of standards in the IoT ecosystem poses interoperability challenges between devices and systems. Improvements in sensor and algorithm accuracy are also needed so that the system can be relied upon for clinical decision-making in various contexts. In the future, the development of IoT in health monitoring is believed to be further enriched by the integration of Artificial Intelligence (AI) and Machine Learning (ML) [6], [18], [21], [28], which can generate personalized insights from health data, strengthen medical predictions, and accelerate diagnosis. Mobile devices are also expected to remain the primary interface, enabling patients to be more active in managing their health. Literature related to IoT Applications and Benefits in Healthcare A literature review shows that IoT makes a real contribution to improving the quality of healthcare services through the integration of more accurate sensor data. IoTbased monitoring systems enable real-time measurement of vital parameters—heart rate, body temperature, oxygen saturation. In one study, the communication range of IoT devices reached approximately 31 meters, four times wider than the Fitbit Charge 3 BLE, with promising clinical accuracy [5]. Additionally, a multi-sensor framework for Body Sensor Networks (BSN) has proven effective in promoting more proactive health management [1]. Furthermore, IoT expands access to healthcare services in rural areas that are difficult to reach with conventional facilities [7]. Thus, this technology strengthens the healthcare system through more accurate, responsive, and inclusive monitoring. Real-Time Health Monitoring with Wearable Devices Table 2. Literature related to Wearable Devices for Health Monitoring Wearable devices equipped with sensors now play a central role in real-time health monitoring. This technology allows patients to monitor their physical condition anytime and anywhere. MICHAEL KURNIAWAN 6 Study Case Studies show that wearable systems can achieve up to 95% accuracy in measuring oxygen saturation and heart rate, equivalent to commercial medical oximeters [14]. In addition, data from various body sensors can be combined to produce a more comprehensive picture of health [10]. In an emergency, these devices can also trigger automatic alerts when vital parameters exceed normal limits, allowing medical personnel to respond more quickly [10]. Literature related to IoT-based Remote Monitoring IoT has driven a revolution in remote health monitoring by enabling unlimited access to medical services, especially for people in remote areas. Through wireless sensor architecture, health monitoring can be done in real-time without having to be at a physical health facility. A concrete example is shown in IoT architecture for emergency situations that accelerate medical response and improve patient safety [3]. In blood pressure measurement, this system achieves an average accuracy rate of 96.26%, with a small margin of error in systolic and diastolic pressure as well as body temperature [16]. Additionally, its application in the workplace has improved worker safety, especially in high-risk areas [6]. Literature related to the Reliability and Accuracy of IoT Systems The reliability of IoT in early detection of health problems has been proven to be significant. Cloud-based systems that integrate wearable devices have proven to be superior in terms of scalability and data accuracy [8]. This implementation enables the identification of health problems at an early stage, thereby preventing serious complications [20]. In rural areas, cloud-based IoT systems have also proven to be effective in providing quality health services [21]. Several studies show that the accuracy rate of patient health status classification reaches 89.96% [18]. These findings confirm the great potential of IoT in improving disease prevention and early detection. Security, Privacy, and Standardization Challenges Table 5. Literature related to IoT Security and Privacy Despite its promise, the application of IoT in health monitoring still faces serious issues related to security, privacy, and standardization. The large volume of patient data increases the risk of information leaks, especially if transmitted over unsecured networks [18]. To overcome this, monitoring systems have begun to integrate advanced encryption and multi-layered authentication [19]. However, more comprehensive interoperability standards are still urgently needed for IoT systems to be widely adopted in the US healthcare sector. Recent studies have even reported accuracy rates of over 95% in various IoT system tests [26], demonstrating great potential for continued development. MICHAEL KURNIAWAN 8 Study Case CONCLUSION AND RECOMMENDATIONS This literature review confirms that IoT has great potential to revolutionize modern healthcare services. Innovations in wearable devices, cloud platforms, and analytical algorithms have enabled real-time physiological data collection, remote monitoring, and early detection of various health problems. The implementation of these systems has proven to be highly accurate, helping to effectively detect critical conditions and expand access to services for previously underserved populations. However, a number of challenges need to be addressed urgently, including data security, interoperability between systems, and clinical validation. Future research should focus on: 1. Strengthening patient data security and privacy. 2. Developing global interoperability standards. 3. Conducting comprehensive clinical trials to validate the system's effectiveness. With the support of AI, ML, and EHR integration, IoT has the full potential to transform healthcare management, creating more proactive, efficient, and patientcentered services.
