Blood Product Administration Case Study I. Mr. Thomas is a 55-year-old male you are caring for this shift after his surgery. The doctor on call informs you that they would like to transfuse Mr. Thomas with 1 unit Packed Red Blood Cells (PRBC). Describe the steps of this process you perform to give Mr. Thomas the ordered blood product. To begin the transfusion, verify the doctor’s order for 1 unit of PRBCs and confirm that Mr. Thomas has given informed consent. ?Check that his blood type and crossmatch are complete to ensure compatibility. ? Gather all necessary supplies, including blood administration tubing with a filter, a bag of 0.9% normal saline, an IV pump, and gloves. ? Before retrieving the blood from the blood bank, ensure Mr. Thomas has a functioning IV line. ?Once the blood product is obtained, verify it with another nurse by cross-checking the patient’s ID, blood type, and compatibility, as well as the expiration date of the blood product. ? Perform a baseline assessment of Mr. Thomas, including his vital signs (heart rate, blood pressure, temperature, respiratory rate, and oxygen saturation), and review his history for any prior transfusion reactions. ? Start the transfusion by connecting the blood unit to the administration set and beginning at a slow rate (2 mL/min) for the first 15 minutes while closely monitoring for any signs of a reaction, such as itching, fever, or chills. ? Reassess vital signs after the first 15 minutes, and if there are no issues, increase the transfusion rate to ensure the unit is completed within 4 hours. ? Continue to monitor Mr. Thomas’s vital signs at regular intervals and document all steps, including the start time, transfusion rate, and his response throughout the process. II. Half way through the PRBC transfusion you assess Mr. Thomas and do another set of vital signs. They are: HR 102, BP 105/59, and T 38.6F. Your patient is now complaining of being itchy and you find raised red bumps on the patient’s arms. What do you do next? Describe the steps that you will take after this assessment. The first step is to immediately stop the transfusion to prevent the reaction from worsening. Disconnect the blood tubing, flush the IV line with normal saline to maintain IV access, and assess Mr. Thomas for other symptoms like difficulty breathing or chest pain. Take a full set of vital signs again to compare with previous readings and evaluate the severity of the reaction. ? Next, notify the on-call doctor and report the reaction, providing details of Mr. Thomas’s symptoms and vital signs. Contact the blood bank so they can investigate the blood product for potential issues. The doctor may prescribe medications such as diphenhydramine (an antihistamine) to relieve itching, acetaminophen to lower the fever, or corticosteroids for more severe reactions. Administer these medications as ordered and continue monitoring Mr. Thomas closely to ensure his symptoms improve. Document everything, including the symptoms, the time the transfusion was stopped, any medications administered, and his response to the interventions. ? Finally, return the blood bag, tubing, and any associated labels to the blood bank for further analysis, as per hospital protocol.
