Vision: to provide safe and high-quality care to our patients through specialized treatment.  Services provided: inpatient and outpatient care that entails maternity, cardiac catheterization, comprehensive surgery, stroke, and emergency care treatment. IDENTIFIED PROBLEMS (Item 2)   11,500 admissions per year and 131,000 outpatients per year. High incidences of reported medical errors  High readmission rate  Medical errors: avoid diagnostic errors; make EHR systems more interoperable, SOLUTIONS (Item 3) work to reduce infections, getting pharmacists more involved in patient treatment, and adopting a structure of coordinated and collaborative conversations for continuity of patient care.  Readmission rate: understanding which patient population is at risk for high readmission, ensuring patients have a 7-day follow-up schedule, ensuring a smooth transition of care, from registration to discharge, communicating written post-discharge instructions clearly , and effective ratio of clinical MEASURABLE PROJECT GOALS (Item 4)   staffing for inpatients and outpatients during the course of patient care. Improve patient education by October 1, 2020. Implement technologies appropriate for reducing medical errors by September 15, 2020.  Strengthen patient follow-up process by August 31, 2020.  Implement employees on-the -job-training to on new technologies to reduce medical errors by August 30, 2020.  Obstacles: employees’ resistance to change, patients’ failure to cooperate, budget OBSTACLES AND OPPORTUNITIES (Item 5) constraints, and timeline.  Opportunities: reduce mortality rate, lessen the re-admission rate, and improve patient experience, expanding the facility services, learning the use of the latest technologies, applying evidence-based and best practices.
