Medications used to treat severe preeclampsia include: Antihypertensive drugs to lower blood pressure Labetolol 200-2400 mg PO Hydralazine 50-300 mg PO HCTZ 12.5-50 mg PO Anticonvulsant medication, such as magnesium sulfate, to prevent seizures. Magnesium Sulfate 40g in 1000mL of LR Nursing Dx: Risk for seizures related to preeclampsia as evidenced by hypertension, proteinuria, edema, changes in vision, severe headache, pain in upper belly, N/V, and increased liver enzymes. Lab Expected Patient RBC 4.5-5.5 4.1 mil/mm3 Pt has urine protein 1+, pedal edema 3+ and facial edema. 3 WBC 5000-10,000 7200 / mm hgb 10-15 (preg) 16 g/dL hct 36-44% 37% platelets protein 150k-400k 6-8.3 158,000 /mm3 7.0 g/dL albumin 3.4-5.4 4.0 g/dL bilirubin 0.1-0.2 0.3 mg/dL AST 8-33 75 units/L ALT 4-36 38 IU/L ALP 20-130 220 IU/L Diagnostic Procedures - BP - UA or 24-hour urine collection - CBC - Liver function panel - Ultrasound Preeclampsia. 28 yo female, G1P0, at 27 weeks gestation. Patho is complex but is thought to be caused by the placenta not developing properly due to a problem with the blood vessels supplying it. Defective invasion of the spiral arteries by cytotrophoblast cells is observed during pre-eclampsia. Goals: 1. Within 24 hours after intervention, pts BP will be below 140/90 2. Within one hour of diagnosis, seizure precautions will be in place. 3. After diagnosis, explain importance of measuring intake/output. 4. Fetal HR will stay within normal limits. 5. Pt will verbalize importance of diet. 6. Educate pt on the importance of bedrest. S/S include HTN, proteinuria, severe headache, vision problems, pain just below the ribs, nausea, vomiting, and sudden swelling of the face, hands or feet Pt had a BP ranging from 162/98162/114 Pt c/o epigastric pain, blurry vision, N/V, and severe headache with pain 8/10 Interventions: 1. Administer antihypertensives as prescribed and check BP every q2h 2. Administer Magnesium Sulfate as prescribed. Pad bed rails, place bed in lowest position, and have necessary equipment at the bedside. 3. Measure strict I&O to monitor kidneys. 4. Monitor FHR q2h. 5. Educate pt on limiting Na and taking Ca, Mg, and K supplements prevent the progression of edema and hypertension in preeclampsia. 6. Educate pt that she should remain on reduced activity with frequent rest periods lying on her side to improve blood flow to the placenta.
