NURSING PROCESS (AAPIE) Must Know! Must Do! • Assessment • Ask Questions • Review vitals, lab values, and other diagnostics • Hands-On Assessment (focused) • Analyze • PLAN • PRIORITIZE! (ABCs, what is the most concerning? The other issues can WAIT!) • Make sure to do non-pharm actions, meds, consults, teachings, other diagnostics • Implement • Least Invasive to Most Invasive (unless it is true emergency) • Evaluate (go back to Assessment) For Example: Assessment – pain 8/10, constipation, fever 102F (fever is the most concerning) Analysis – possible infection Plan – ice pack, cooling measures, antipyretics, infection control consult, teach about infection control, ask MD for CBC/blood cultures/etc. (ONLY PLAN FOR THE PRIORITY! THE OTHER ISSUES CAN WAIT!) Implement (least invasive 1st) Evaluate (if not resolved, go back to AAPIE) REPEAT AAPIE FOR THE OTHER ISSUES! Each problem is its own AAPIE! NURSING PROCESS (AAPIE) Must Know! Must Do! HOW TO DOCUMENT – Follow AAPIE! Charting MUST have 3 components! Follow AAPIE! 1st component – must chart the ASSESSMENT 2nd component – chart your IMPLEMENTATION 3rd component – chart your EVALUATION For Example: 0900 – pt has fever 102F (Assessment) 0910 – ice pack applied, other actions (Implementation) 0930 – temp is now 99F (Evaluation) NURSING PROCESS (AAPIE) Must Know! Must Do! Look Up Medications! Question MD orders if needed! DO NOT JUST OBEY THEM! Before Giving Meds, FOLLOW AAPIE! • • • • • What assessment should you look for before giving that med? Analyze your findings Plan if med should be given or not! Implement (give it nor call the MD for different med) Evaluate For Example: MD orders Tylenol for fever • What assessment should you look for before giving that med? o Assess if patient has LIVER Problems! If can swallow, if risk for aspiration, if allergic, assess how much Tylenol they received in 1 day? Etc. • Analyze your findings o Based on assessment, they do have Liver problems • Plan if med should be given or not! o Must not give this med! • Implement (give it nor call the MD for different med) o Call MD for different antipyretic that is safer • Evaluate Now MD ordered Ibuprofen for the fever since Tylenol is not safe…. Repeat AAPIE as seen above! MUST DO AAPIE before giving ANY MEDS!!! Be SAFE! ADVOCATE for your patient! AAPI AAPIE E ALWAYS!
