1 PROCEDURE CHECKLIST Chapter 24-2: Bathing: Providing a Towel Bath Check (ü) Yes or No PROCEDURE STEPS Before, during, and after the procedure, follows Principles-Based Checklist to Use With All Procedures, including: Identifies the patient according to agency policy using two identifiers; attends appropriately to standard precautions, hand hygiene, safety, privacy, body mechanics, and documentation. 1. Places folded bath blanket, bath towel, and two or three washcloths in large plastic bag. 2. Fills pitcher with approximately 2,000 mL of warm distilled, sterile, or filtered water; adds nonrinse soap or commercial solution to the water; and pours solution into the bag over the bath blanket, towel, and washcloths. 3. Uses warm, not hot, water (105°F [41°C]). 4. If not planning to change the linen, works one dry bath blanket under patient. 5. Places a dry bath blanket over patient and pulls down bed linens, without exposing patient. 6. Removes patient’s gown without exposing patient. 7. If patient has an IV, removes the gown first from the arm without the IV; replaces gown on the affected arm first. (If the IV tubing has a two-way needleless connector that maintains a closed line, it is acceptable to disconnect the tubing to remove the gown.) 8. Replaces the dry blanket with a wet blanket: a. Squeezes out excess water so it does not drip. b. Pushes the dry blanket down to patient’s waist and places the wet blanket on the chest; continues to unfold wet blanket in this manner, keeping the dry blanket dry. 9. If necessary, places another dry blanket on top of the wet one. 10. Bathes patient beginning at the feet and working toward the head. 11. Uses the wet blanket to wash legs, abdomen, and chest. As bathing is done, replaces the wet blanket with the dry one, folding the wet blanket as each area is bathed to allow only clean surfaces to contact new body areas. 13. Uses one washcloth to wash patient’s face, neck, and ears. 14. Dons procedure gloves (if not already wearing them). Rolls patient to one side and uses a wet towel to wash the back and then the buttocks. 15. Washes the rectal area; removes any fecal matter with tissues before using the washcloth. 16. Changes procedure gloves; washes hands. 17. Using washcloths, provides perineal care. 18. Supports joints when bathing. 19. Pats dry with dry towel, as needed; dries thoroughly between the toes. 20. Applies deodorant, lotion, and/or powder as desired or as needed. 21. Provides a back rub if not contraindicated. 22. When finished, repositions patient, and changes bed linen as needed.
