Endemic RF: B. duttoni, B. hermsil & B turicati Bytick Pathogenex's M07: Vect or Borne (Pediculus humanus) Epidemic RF: transmited & Human Body louse Scratching or dep. of spirochekes on abraded skin or mucosa Endemic RF transmitted by bite of an Infected tick (Ornithodorus species) Spread host tried to eliminate; borelid surface antige intia hence evado Undey repeated antigenic Variatia. host immune system leading to repeated bactermia & recurrent febrile episodes Clinical manifestation Incubation Reriod = 7-8 days • Recurrent febrile episoda lasting for 3-5 days occur intervening with affebrile episodes 4-14 days. Relapse can ocar with 10 times. Hominhage: Petechiae s Sputem epistaxis and Blood hinged ore more liky in epidemic RF. • Neurol o gic Factor - meningitis, seizure, Focal deficits, paraplegia, & psychosis may occur in 30% cases. LABORATORY DIAGNOSIS Microscopy -poriphorul thick or tin smear by wright or Giemsa stain. - Direct Fluorescent antibody test using monoclonal Ab is used to identif the species. -Dirk grond microscopy to demonstrate motile Spirochetes. - Pour grom staining. CULTURE afebrile, fails to detet //.... Isolate from blood :- Confir netin SEROLOGY→ -BELISA & IFA (Indirect Fluoresume arsy) to detect serum Abs. B PCR delcy 165rRNA & alpa genes to ides various sps. of Burellic CA RF.//
