Immunisation & Screening Schedule Day 1 Baby check Vit K Hearing screen (OAE) Advice to parents Identification of vulnerable children Within 10 days Heel prick card (GUTHRIE) Further advice on weight etc 2 Month DTaP + HiB + Polio PCV 3 Month DTaP + HiB + Polio Men C 4 Month DTaP + HiB + Polio PCV Men C 12 Month HiB + Men C 13 Month MMR PCV Pre-School DTaP + Polio MMR 4-5 yrs Visual screening Dental check Primary 7 Dental check School (13-18) Dip, Tet + Polio BCG Immunisation if: • Familiy Hx TB • Likely to travel to high risk area • From immigrant country with high risk • Parents request it • NB: High Risk = Incidence = > 40 / 100,000 Nature of Vaccines Live Vaccines (not in immune suppression): BCG Polio (oral and sabin) – NOT Salk MMR Yellow fever Wait for 3 months after imm sup stopped Non-Live Vaccines DTaPP (toxins) and Influenza IM polio is inactive • SALK is inactive therefore safe in immune suppression = component of DTaPP • Given IM / SC • Don’t correct timings for prems HiB & Pneumococcus = subunit vaccines In HIV Patients Contraindicated – BCG, Oral Typhoid & Yellow Fever CAN have MMR & Zoster if not significant immune impairment Contraindications Measles if: Allergy to vaccine / gelatine / neomycin Pregnant Acute febrile illness Immune suppression MMR if: Allergy to neomycin / kanamycin Previous severe reaction Untreated cancer / immune problems Postpone MMR if Acute febrile illness Another live vaccine within 3 weeks Administration of Ig within 3 months Flu vaccine – avoid if egg anaphylaxis Pertussis – contraindicated if progressive neuro disease (ok once stable) Vaccines for Splenectomy Standard course of childhood imms Ensure capsulated organisms HiB, Meningococcus & Pneumococcus Give 2/52 prior to elective splenectomy Worldwide Tetanus is still common Diptheria is still common Polio is near eradication Smallpox is eradicates AIDS in increasing
