International Classification of Function & Rehabilitation ICF is similar to ICD but looks at mortality, psychosocial & biological aspects Health Condition (disorder/disease) Body Structure/Function (impairment) Activity (limitation) Environment Participation (restriction) Personal Body Function – Physiological & psychological functions (e.g. breathing, thinking, walking) Body Structure – Anatomical parts (e.g. lungs, brain, legs) NB: loss of body structure has varying effect based on age, lifestyle etc Activity – e.g. walk, cook, drive – if impaired = limitation / disability Participation – interacting, living as you want, involves roles (parents, carer, worker). If impaired = restriction / handicap (e.g. inability to work, live at home) Environment = can be physical / social (e.g. wheelchair, ramp, lift, social security benefit) Personal = some people wish to be independent, even if that means a struggle Domains of health in Rehabilitation Body Structure & Function Goal setting Encouragement Behaviour change Overcome fear Information Environment Physical, social, legal Co-operation – so can participate Assistance – with problem tasks Legal - prevent discrimination Societal - support Capacity Activity Participation Rehab (physio, exercise) Treat barriers to rehab (see later) Information giving Skills training (learn an alternative) à restore & maximise function Aids & Appliances Adaptations e.g. tipping kettle, walking stick, ramps Personal Enquire about roles: Aim to restore/sustain (e.g. work / stay at home) NB: diagnosis has little role in rehabilitation – it is the structure, function, capacity and acitivity you are treating, NOT the disease Barriers to Rehabilitation Inadequate disability management (e.g. bed sore)/ assessment / planning / conditions Iatrogenics – review meds Pain Depression (1/3 of all stroke patients) Medical – CVS (angina, HF), RS, UTI, DVT, PE, constipation, faecal impaction, metabolic
