SOAP Note Assignment Athlete’s Name: Tsidro Requejo Body Part Injured: Left Hip Sport: Soccer Subjective Information Mechanism of Injury: Athlete was running and felt popping. Chief Complaint: Pain and popping with running and stretching Pain: 5/10 Objective Information Observation: No swelling, deformity or bruising. Range of Motion: Tested flexion, extension, and abduction. There was pain with extension. Strength: Weak Special Test: Thomas and Fabers Tests Assessment Disposition: Snapping hip syndrome List of problems: Pain, popping in hip iliopsoas, tightness Plan of Action Initial Treatment: Ice, Stretch The athlete will be treated by a certified AT. Treatment will be 5 days per week for 2 weeks. Treatment will consist of: Stretching, pain management and reduction of inflammation. Short term goals: Full ROM without pain Intermediate goals to be completed in 2 weeks: Equal Bilateral Strength
