Nursing Diagnosis Acute pain related to infection in the urinary system due to bacterial infection. Goal: Administer pain medication and monitor patient hourly until pain level is manageable. Medical Diagnosis Pathophysiology Urinary Track Infection Upper UTI- affects kidney (renal) Lower UTI affects the urethra and bladder. UTIs are generally caused by bacteria but can also result from certain viruses and fungi. Mainly noted symptoms include burning sensation during urination, frequent urge to urinate, and pain in the back or lower abdomen. Patient Data 78 year old female lives, at home, hx of type two diabetes Diagnostic Data Medical Interventions Urinalysis, blood cultures, STD test, CT scan, ultrasonography Placed on oxygen, raise HOB, placed on antibiotics, IV fluids, vasopressors, monitor intake and output, place on fall precautions. Nursing Interventions Relieve pain: Antispasmodic agents may relieve bladder irritability and analgesics and application of heat help relieve pain and spasm. Push Fluids: encourage to drink lots of fluids. Voiding: encourage voiding every 2-3 hours. Irritants: avoid urinary irritants such as coffee, teas, sodas, alcohol Patient Education Wipe from front to back, proper hand washing, unrestrictive underwear, finish the full course of antibiotics, drink plenty of fluids, can be accompanied by yeast infection, counteracts birth control.
