ACTIVE LEARNING TEMPLATE: Medication Levothyroxine MEDICATION __________________________________________________________________________ REVIEW MODULE CHAPTER ___________ Thyroidnhormone CATEGORY CLASS ______________________________________________________________________ PURPOSE OF MEDICATION Expected Pharmacological Action Thyroid hormones are synthetic form of thyroxine T3 a form of liothyronine T3 or a combination of T3 and T4 that increase metabolic rate, protein synthesis, cardiac output, renal perfusion, oxygen use, body tempreture, blood volume and growth process Therapeutic Use Thyroid hormone replacement is used for treatment of hypothyroidism(all ages and form). Thyroid hormones are used for the emergency treatment of myxedema coma (IV route), a severe deﬁciency of thyroid hormone. Also used for cretinism and simple goiter. Complications Overmedication Chronic overtreatment Medication Administration IV Contraindications/Precautions Pregnancy risk category A. Pregnacy may require increased dosages.Use is contraintraindicated for clients who have thyrotoxicosis and adrenal insufﬁciency. Because of cardiac stimulant effects use is containdicated following a MI.Use cautiously in clients who have cardivascular problems (hypertension, angima pectoris, ischemic heart disease). Use cautiously in older adults who can requirw lower dosages. Nursing Interventions Obtain baseline vital signs weight and height and monitor periodically throughout treatment. Interactions Increases cariac responsiveness to catecholamines epinephrine dopamine dobutamine thereby increasing the risk of dysrhythmias Can increase requirements for insulin and digoxin Binding agents antiulcer medications,calciumnand iron supplemets, magnesium salts and foodnreduce levothyroxine absorption with concurrent use. Client Education Take medication daily on an empty stomach 30 to 60 minutes before breakfast. Evaluation of Medication Effectiveness Depending on therapeutic intent, evidence of effectiveness can include the following: Decreased TSH levels. Evaluation of TSH should not be done until 6 to 8 weeks following the start of treatment. T4 levels within expected refrence range Absence of hypothyroidism manifestations
