To collect information on the health status of the patient
Indications
Obtaining information about the physical
health of the patient, including establishing
baseline status and detection of health
abnormalities.
Outcomes/Evaluation
he patient will begin to develop a therapeutic
relationship with the nurse, and feel
comfortable voicing concerns. The nurse will
collect adequate information to determine
the current status of the patient's health and
any detriments to health

CONSIDERATIONS
Nursing Interventions (pre, intra, post)
Introduce self to patient, perform necessary
hygiene, provide privacy. - Explain process
of physical assessment - Ask permission
before performing any physical assessment,
including if the patient has any concerns or
religious/cultural requirements. - Describe
each action before it is performed.
Client Education
Educate patient on assessment process
including what it will entail and what will be
done with the information, including health
promotion and preventive care.
Nursing Interventions
Discuss patient's concerns and educate
patient on assessment process. Alterations
of assessment may be necessary if patient
is unwilling to allow physical assessment
