Case Study, Chapter 12, Antiprotozoal Agents An adult patient and two school-age children have recently returned from 2 months in Africa. While in Africa, the patient misplaced the preventive medication for malaria. (Learning Objectives 1, 2, 3, and 5) a. What is the life cycle of the protozoan that causes malaria? a. The malaria parasite life cycle involves two hosts. During a blood meal, a malaria-infected female Anopheles mosquito inoculates sporozoites into the human host. Sporozoites infect liver cells and mature into schizonts , which rupture and release merozoites. After this initial replication in the liver, the parasites undergo asexual multiplication in the erythrocytes. Merozoites infect red blood cells. The ring stage trophozoites mature into schizonts, which rupture releasing merozoites. While in the mosquito’s stomach, the microgametes penetrate the macrogametes generating zygotes. The zygotes in turn become motile and elongated which invade the midgut wall of the mosquito where they develop into oocysts. The oocysts grow, rupture, and release sporozoites, which make their way to the mosquito’s salivary glands. Inoculation of the sporozoites into a new human host perpetuates the malaria life cycle. b. What is the goal of treatment for malaria? a. The goal is to clear all parasites from the body and to reduce the transmission of infections by reducing the infectious reservoir. Also, the preventing of emergence and spread of resistance to antimalarial medication. c. What are the common adverse reactions of drugs used to treat malaria? a. Headaches, body aches, weakness, nervousness, irritability, dizziness, confusion, seizures. d. What other common protozoal infections, including cause and clinical presentation, could this patient develop from the trip abroad? a. The patient who is infected with malaria is likely to complain of fever, chills, vomiting, cough, diarrhea, and abdominal pain. e. What are the key nursing considerations for the adult patient and two schoolage children who have been receiving an antiprotozoal agent? a. The nurse should take into consideration that the adult patient may be pregnant. Allergies to medications and ages of children are important considerations as well.
