Using Feedback on Patient Health Outcomes to Improve Orthopedic Physical Therapist Practice: Progress Report Background and Purpose Methods • Achieving the Quadruple Aim in health care requires a commitment to evidence-based practice (EBP) focused on improving the quality and safety of patient care, while reducing costs and engaging clinicians for increased job satisfaction (Beckett & Melnyk, 2018). • Quality improvement (QI) in health care has focused on aligning the latest research with clinical practice and patient outcomes (Melynk & Fineout-Overholt, 2017). • Clinically important outcomes should be part of routine physical therapy (PT) practice; however, the lack of literature suggests that QI using outcomes may not be widespread in rehabilitation. • The PDSA cycle is the most commonly used tool in quality improvement programs within health care (Ivers, et al, 2012) • The purpose of this feasibility study is to develop and test a QI program that uses a systematic process of plan-do-study-act (PDSA) cycles based on the outcomes of patients with low back pain and neck pain. • PT clinics were recruited from the University of Pittsburgh Medical Center (UPMC) Centers for Rehab Services (CRS) to complete QI training and design and implement QIPs. • QIPs will be directed to improve care for patients with low back or neck pain using the Institute of Healthcare Improvement Model PDSA cycles. • The Oswestry and Neck Disability Scores were selected as mandated outcomes however clinics were allowed to choose other measures. • Figure 1 presents current progression through program phases. Study design • A mixed-methods design will be applied to develop and test the feasibility of a quality improvement program (QIP) in outpatient orthopedic PT practices that includes asynchronous training for PTs on applying a QI process. Study Methods and Progress Asynchronous Online Training Modules • Asynchronous, online module included foundations of QI and learning health systems, IHI Model for Improvement, and PT-specific cases of outcomes based QIP. • Designed for active learning with knowledge checks and case-based problem solving corresponding to each phase of the PSDA cycle • CEU credited for Pennsylvania PTs Progress to Date Phase I • Literature review informed module design and completion • Recruitment of three QIP teams (n=17) • All Participants completed asynchronous, online training • All Participants completed consent form and questionnaires Phase II • Champions selected from each QIP team • QIP teams provided outcome reports • QIP teams currently in the “Plan” phase of PSDA cycle: selecting QIP objectives based upon outcomes data • Final decisions and implementation on hold due to current health crisis and decrease in patient volume across all clinics
