An Optical Device for the Detection of Counterfeit and Substandard Antimalarial Drugs Counterfeit and substandard antimalarial drugs are a catastrophic global health challenge with widespread repercussions. However, the problem has gone unnoticed because countries affect-ed by malaria are often middle or low-income and largely ignored. Previous methods to identi-fy counterfeit and substandard drugs have been expensive, not portable, and difficult to use in low-resource settings. To address this global health challenge, a design for a low-cost, portable, and user-friendly quality-assurance optical device using elastic scattering in the UVvisible-NIR wavelength range and fluorescence spectroscopy was developed, to detect lowquality and coun-terfeit antimalarial drugs. Two pill models were used in the lab setting to test various optical spec-troscopy techniques and to determine which ones best serve as the basis for the device design. These two models consist of pills that contain the incorrect active pharmaceutical ingredient (API) and stressed antimalarials exposed to extreme environmental conditions to mimic counterfeit and substandard antimalarial pills, respectively. Both elastic scattering spectroscopy (ESS) and fluo-rescence spectroscopy, were experimentally determined as viable optical methods for use in the proposed device as they have the capability to differentiate between standard pill samples and substandard or counterfeit pill samples. The proposed device was designed to use both ESS and fluorescence in conjunction to function as a two layer differentiation system. The device provides a low cost, portable, and user-friendly method that serves to improve drug quality detection in middle and low-income countries, reducing the ramifications of counterfeit and substandard drugs received by patients suffering from malaria.
