ACTIVE LEARNING TEMPLATE: Diagnostic Procedure brain natriuretic peptide (BNP) PROCEDURE NAME ____________________________________________________________________ REVIEW MODULE CHAPTER ___________ Description of Procedure Measures the amount of the BNP hormone in your blood. BNP is made by your heart and shows how well your heart is working, as well as how much the ventricles are "overstretching" as a result of HF. Normally, only a low amount of BNP is found in your blood. Indications BNP levels help your doctor figure out if you have heart failure or something else that has similar symptoms such as shortness of breath. The test also shows if your heart failure has worsened. It'll help your doctor decide what treatments you need and if you need to be hospitalized. Interpretation of Findings -NP levels go up when the heart cannot pump the way it should. A result greater than 100 pg/mL is abnormal. The higher the number, the more likely heart failure is present and the more severe it is. Sometimes other conditions can cause high BNP levels. -Generally, BNP levels below 100 picograms per milliliter (pg/ml) are considered normal. Levels above 400 pg/ml are considered high.- Potential Complications Patient is recieving nesritide may have transiety and significantly elevated BNP levels at the time of administration and must be interpreted with caution CONSIDERATIONS Nursing Interventions (pre, intra, post) -Treatment with angiotensin-converting enzyme inhibitors, angiotensin-II receptor blockers, spironolactone, and diuretics reduces BNP levels, suggesting that BNP testing may have a role in monitoring patients with heart failure. -Let patient know they need to be NPO for 4hrs before the test as well Client Education Explain to the patient that BNP is a neurohormone secreted by the heart in response to expansion of ventricular volume and pressure overload. It helps maintain the body's salt and water balance in the face of the neuroendocrine stimulation that occurs with heart failure. Nursing Interventions -Limit fluid as appropriate. -Assess for peripheral edema, JVD, adventitious lung sounds such as crackles. -Monitor blood pressure, heart rate, and intake versus output. -Administer prescribed diuretics, restrict sodium intake, and order a low-sodium diet.
