Examples of the 8 HPI Elements Onset - When did it start? Examples: 7am today, yesterday, 6 years ago…? (Note: Do not use an onset of the day of the week, like “started Monday” - this is too vague!) Timing - How often is it present? Examples: All the time (constant)? Comes and goes (intermittent)? Always there but intensity changes (waxing and waning)? Location - Where is it (be specific)? Examples: Right shoulder, left leg, head, mid-back? Quality - What does it feel like? Examples: Stabbing, cramping, achy, throbbing, pressure? Severity - How bad is it? Examples: 7/10 on a 1-10 pain scale? Horrible (severe)? Or mild? Associated symptoms - Did they develop any secondary symptoms? Or, did the CC cause any other symptoms? Example: “My stomach started hurting AND NOW I’m throwing up” - vomiting is an associated symptom. Pertinent negatives - anything they specifically say they do NOT have? Example: “I am having vision changes but my head DOES NOT hurt.” - NO headache is a pertinent negative (sometimes this information is given by the patient in response to a question the provider asks). NOTE: Associated symptoms and pertinent negatives count together as ONE element. Modifying factors - What have they tried? Did it change the symptoms (help or hurt)? Example: “I have a terrible backache. Lifting weights made it a lot worse yesterday. Hot packs made it better. I tried Tylenol last night and it didn’t help at all.” - Weight lifting, hot packs, AND Tylenol are all modifying factors. Context Is there anything else that stands out to you/anything else important? Examples: “I have headaches occasionally...also, I drink 12 cups of coffee every day.” OR “I have headaches and I have previously been diagnosed with migraines.” Complete Single Complaint HPI Example Ms Winchester is a 48y/o F complaining of a headache which started two days ago. She describes her headache as a throbbing pain that is constantly present in her R temple, rated a 5/10 in severity. Tylenol last night did not change her symptoms, but reports improvement with rest in a dark room. She has some associated dizziness but denies any nausea/vomiting. Reports no recent head injuries and has never experienced this type of HA before.
