1 Performance Assessment 1 December 2, 2020 2 Performance Assessment 1 Impact of the Problem on the Patient The topic of concern that I feel is one of the most troublesome in healthcare today is nurse burnout. Nurse burnout can ultimately lead to compassion fatigue, which results in impaired judgement and lack of attention to detail. This results in neglected care of the patient which can lead to unsafe conditions, medical mistakes, and poor patient outcomes. Impact of the Problem on the Organization Nurse burnout can result in harm to patients, and this can lead to costly and legal consequences. Patients or their family members have the right to file lawsuits against hospitals and healthcare facilities for medical malpractice or negligence. If won, the hospitals and facilities must pay fines to the state board, the family involved, as well as other accreditation agencies. This has the potential for the hospital to lose certain accreditations that it has as well. This without a doubt can have a negative impact on a hospitals reputation throughout the state and community, which can result in a loss of business by elective patients in non-emergent situations. Identify the PICO components P – Nurse Burnout I – Mindful and evidence-based training C – Lack of training O – Reduction of amount of nurse burnout in the hospital Evidence Based Practice Question With response to nurses, does the act of implementation of mindful and evidence-based training, compared to lack of or no training, reduce the amount of nurse burnout in a hospital? Research Article 3 Background Introduction The aim of this article was to study and analyze the efficiency of mindful evidence-based training and how it affects the amount of nurse burnout that nurses can develop. Methodology This study was a meticulous review of multiple other scientific studies. The researcher referred to these databases: LILACS, CINAHL, Medline, PsycINFO, ProQuest (Health and Medical Complete), Scielo and Scopus. The writer also used the PRISMA Statement guidelines and finished the research in June 2019 using a search equation. Level of Evidence 4 The level of evidence associated in this article is level III as it is a systematic review and meta-analysis research that was completed using Quasi-experimental and Randomized control trials. Data Analysis The difference of the means that was used to discover and accumulate the information in the data groups using the T-Test. Ethical Considerations There was not any ethical consideration to be noted or approval from a board review as it was a systematic and methodical review of the past studies. The researcher did not have any human participants and therefore the need for a consent and ethical review was not needed. Quality Rating For this article, the quality rating was an A as the researcher had completed a Quality Appraisal review. The author verified the reliability of the methodology, while checking for any possible bias, and also had transparency while reviewing the data. The author used the Oxford Center for Evidence-Based Medicine to classify the level of evidence in the studies. They also used the Cochrane Collaboration Risk of Bias Tool by two different independent peer reviews to look for any possible bias. Lastly, the Cohen’s Keppra test was used to verify the reliability of the data coding manual. Analysis of the Results / Conclusions This article concludes revealing how mindful evidence is an effective intervention that can reduce nurse fatigue and burnout. The study resulted showing nurses had reduced their level of emotional exhaustion caused from burnout. The article also showed that mindfulness has allowed nurses to have a higher level of personal accomplishments. Background Introduction The purpose of this article was a quality improvement initiative with the use of mindful evidence-based intervention used to decrease the moral distress of the oncology nurses studied. Type of Evidence This article is in the non-research quality improvement category. Level of Evidence This article is a non-research level V quality improvement style article. Quality Rating For this article, the quality rating is a B. This shows that the article has a clear objective with the quality improvement initiative in a specific setting. The article setting is restricted to the oncology unit in question, with the purpose being specific to the full-time RN’s in said unit. Author’s Recommendations At the conclusion of the article, the study showed that mindfulness therapy was indeed an effective exercise that was able to reduce the total amount of nurse burnout in that unit. This exercise showed nurses how to feel supported, empowered, while also allowing a debriefing outlet. The therapy created a space rich with team spirit, strength, and confidence. Overall, this reduced nurse burnout by resulting in a less stressful environment. Recommended Practice Change Based on Evidence Based Practice Question My recommendation of mindful evidence-based practice change is to establish this exercise in a training program as a way to reduce the risk of nurse burnout. Research shows a 6 positive result with most of the mindfulness programs ending in decreased nursing fatigue (Suleiman-Martos et al., 2020). These mindful changes help lead nurses to feel supported and empowered in their current work environment that will evolve into trust and resilience. The new empowering feeling ensures nurses are able to work in a less stressful environment that will have a positive outcome on the quality of care to their patients (Vaclavik, Staffileno, & Carlson, 2018). Key Stakeholders The key stakeholders noted in the article are the bedside nurse, nurse educator, and financial department. The bedside nurse’s role is to be an active participant in the education and programs available while giving feedback to determine to effectiveness of the training program. The nurse educator role is to create the teachings and then implement them into the creation of the program. Finally, the financial department’s role is to distribute funds allowing the project to be created by budgeting as well as applying for grants. Barrier to Implementation Some possible barriers to creating the program could be the financial aspect of said program. The implementation team could possibly be wary of the cost overall including training and supplies. Strategy to Overcome the Implementation Barrier A strategy to overcome the financial barrier could be communicating with the financial department to create a budget for the project and find possible grants to supplement the cost. Indicator to Measure the Outcome A way to determine the program is effective is to maintain communication with the assurance/quality improvement department to evaluate the effects before and after the implantation of the program. The nurses and staff should be able to complete surveys that 7 measure their satisfaction with the new program verses before the program was implemented. The surveys can be collected and compared to determine the staffs’ satisfaction with the program, as well as to discover if there is anything significantly disliked by the nurses. Patients can also be given different satisfaction surveys to acknowledge any improvements with the level of care they are receiving versus before the program was implemented.
