(HEMATOLOGY -4: physiology of hemostasis) (Bleeding Time, Clotting Time, Thrombocyte Count) 1. BLEEDING PERIOD (Bleeding Time/BT Duke way) The bleeding period is an examination to determine the function of capillaries and the function and number of platelets. Principle Calculate the time from when the first bleeding appears until there is no visible trace of blood on the filter paper. Tools & Reagents 1. Blood lancet 2. Filter paper 3. Cotton and 70% alcohol 4. Stopwatch Work Procedures 1) Disinfect the earlobe (CDT) with 70% alcohol, then let it dry. 2) Tension the lobe of the ear between the thumb and forefinger of the examiner's left hand, then prick the lower edge of the CDT with a blood lancet. 3) As soon as blood is visible - start the stopwatch. 4) The blood that comes out almost dripping is touched with filter paper every 30 seconds (be careful not to touch the wound!). 5) Continue doing this until the bleeding stops (there are no more blood spots/traces on the filter paper). 6) Note the time! Reference Value -1 – 3 minutes -3 – 5 minutes (called limit value - border line) -5 minutes (lengthening) note -In situations where the puncture is not deep enough blood may not come out, in this situation don't massage the CDT, but just repeat the experiment on the other ear. -In situations where the bleeding does not stop after 5 minutes, the bleeding is stopped actively by pressing the wound using dry cotton and then plastering it. 2. FREEZING PERIOD (Clotting Time/CT, Modification of the Lee and White method)The clotting period is an examination to determine the presence of abnormalities/deficiency in intrinsic clotting/coagulation factors. Principle Calculate the time from when the first bleeding appears until the blood clots in tube III. Tools & Reagents 1. 3 test tubes measuring 8 X 75 mm 2. Syringe and needle (sterile) 3. Cotton and alcohol 70% 4. Stopwatch Work Procedures 1) First determine the location of the vein from which blood will be taken (usually v. mediana cubiti). 2) Perform proper containment, disinfection, fixation of the vein, then puncture the vein directly. 3) As soon as blood appears to start flowing into the syringe, the stopwatch is turned on. 4) Take a sufficient blood sample. 5) Each tube (I, II, III) is filled with 1.5 ml of the blood sample. 6) Leave for 4 minutes, then start from tube I, tilt tube 90Oevery 30 seconds, to see if there is bloodit's frozen. When it is frozen, record the time and continue in the same way with tube II and tube III. 7) Results: the average value of the three test tubes. Reference Value - 5 - 15 minutes - -15 minutes: lengthening note If there are non-sequential clotting, the results of the blood in the first tube are ignored.
