‭Special shapes‬ ‭Anorexia nervosa in boys‬‭-It constitutes barely 10% of cases of anorexia nervosa. Although her clinical‬ ‭picture is comparable to that of the girl (the loss of all libido and the absence of erection being considered‬ ‭the equivalent of amenorrhea), two notable differences exist: firstly, the hypochondriac considerations are‬ ‭more frequent (preoccupations focused more on the interior of the body than on its external aspect),‬ ‭secondly, it would more often be a possible mode of entry into a form of psychosis.‬ ‭Pre-pubertal anorexia‬‭-It appears when no puberty signs are detectable. This form has‬ ‭some characteristics:‬ ‭-‬‭higher proportion of boys;‬ ‭-‬‭eating disorders present in very early childhood;‬ ‭-‬‭rapid weight loss;‬ ‭-‬‭frequent association with a depressive state;‬ ‭-‬‭seriousness of the prognosis.‬ ‭Late-onset anorexia‬‭-It manifests itself readily after marriage or the birth of the first child. The‬ ‭depressive component is marked there. It seems that most often it follows a discreet anorexic‬ ‭episode during adolescence.‬ ‭Evolution and prognosis‬ ‭Prochaska and DiClemente Stadiums‬‭-They reflect the patient's evolution as they become aware of‬ ‭their condition:‬ ‭Contemplation‬ ‭MEANING‬ I‭ don't need to change, I don't have a problem. It's you who has it.‬ ‭Maybe I have something I would like to change.‬ ‭Preparation‬ ‭I'm looking at what I need to do to change.‬ ‭ ction‬ A ‭Maintenance‬ ‭I am doing what is necessary to change.‬ ‭STADIUM‬ ‭Pre-contemplation‬ ‭I am aware of what I must do to avoid relapse.‬ ‭Weight evolution‬‭-It is considered that approximately 50% of patients return to a normal weight,‬ ‭20% progress favorably while maintaining a weight below normal, 15 to 25% become chronic (which‬ ‭means progression beyond 4 years) and 5% risk developing obesity.‬ ‭Fate of amenorrhea‬‭-70% of patients return to a menstrual cycle with normal possibilities of‬ ‭procreation.‬ ‭The lethal risk‬‭-It is far from negligible: 5% per decade, most often from arrhythmia or suicide (the‬ ‭latter mainly concerns anorexics who have become bulimic). The risk of sudden death becomes‬ ‭notable as soon as weight loss reaches 35% compared to normal, especially since it occurs quickly.‬ ‭Psychological evolution‬‭-At least 50% of former anorexics continue to have psychological‬ ‭difficulties (depressive tendencies, phobias, obsessive-compulsive disorders, loss of interests and‬ ‭contacts).‬ ‭Poor prognostic factors‬‭-These are the appearance of the disease at a later age or the prepubertal‬ ‭forms, the male form with‬‭merycism‬‭, the long duration of evolution, bulimic attacks or‬ ‭induced vomiting, poor response to hospitalization, extreme weight loss and depressive syndrome.‬
