1. PURPOSE This SOP describes the procedure for recording Orthostatic Blood Pressure (BP). 2. SCOPE For Clinical Trials in ECTU or in Primary Care an orthostatic blood pressure recording may be required as part of a screening procedure to determine the eligibility of a study participant. Orthostatic blood pressure recordings may also be required as part of baseline measurements with which to compare those taken subsequently during the course of the trial or as a safety measure during study visits 3. PROCEDURE Orthostatic blood Pressure will be recorded using a semi-automated blood pressure device unless otherwise stated in the study specific protocol. 3.1. Blood Pressure Recording 3.1.1 Equipment required:  Semi-automated blood pressure device 3.1.2 The subject must be in the supine position for a minimum of 5 minutes, check individual study protocol requirements 3.1.3 Attach the blood pressure cuff checking the correct size. 3.1.4 For the smaller adult arm (up to 33cm) use a cuff 12x23cm. 3.1.5 For most adults (up to 42cm) use a cuff 13x35cm. 3.1.6 Place the cuff in the middle of the non-dominant upper arm. 3.1.7 Switch machine on and press ‘start’ to inflate blood pressure cuff 3.1.8 Two consecutive BP recordings should be taken after a minimum of 5 minutes in the supine position, unless otherwise stated by the study protocol. 3.1.9 The recordings should be between systolic BP 150-90 mmHg and diastolic BP 90-60 mmHg with 15 mmHg difference between each of the systolic and diastolic values. 3.1.10 If there is a decrease in systolic pressure 15 mmHg or they are outside the range required by the protocol further recordings may be required. Check individual study protocol instructions. 3.3 Erect BP and P 3.3.1 Immediately following supine recordings ask the subject to stand for 1 or 2 minutes (depending on the protocol) and then take a set of blood pressure measurements as specified by the protocol. The recording should be between systolic BP 150-90 mmHg and diastolic BP 90-60 mmHg, unless otherwise stated by the protocol. If the mean supine systolic BP ≤150 mmHg has a decrease of ≥20mmHg or the diastolic has a decrease ≥10mmHg OR if the systolic is >150mmHg and has a decrease of ≥30mmHg and the 3.3.2 3.3.3 Page 2 of 3 3.3.4 diastolic has a decrease of ≥15mmHg this is considered orthostatic hypotension, ensure the subject is asymptomatic. If either of the standing BP’s meets the orthostatic hypotension criteria as above, then that will be considered positive for postural hypotension. If two out of two or two out of three sequential recordings are positive and orthostatic hypotension is confirmed, record as an Adverse Event. Page 3 of 3
