The technician gathers towels and belongings, then helps the client to the shower. When entering the shower, the client slips and falls to the floor. The technician reports the incident to th nurse. The nurse assesses the client, who denies suffering any other injuries. The nurse documents the assessment, which includes a full set of vital signs, and then notifies the HCP. The nurse knows an adver: event report (incident report) must be completed. Question 17 of 25 Who should the nurse ask to complete the adverse event/incident report? The nurse should complete the full report. This is not the best person to complete the report. The client should complete as much of the form as he is able. The staff is responsible for completing the report. The technician helping at the time of the accident needs to complete the report.
