Antidepressant groups Theory: due to abnormal wl of Ferotimin, merepi, dopamine Selective Serotonin reuptake inhibitors (SSRIs) SSRI's: Fluoxetine - 1st line therapy 4 depressive sx/anatcty disorders MOA: ↑ availability of serotonin Uses: Depression, bipolar II, dyothy thia, anxiety disorder, bulimia. SE's: Sedation, Sexual dysfunction, diarrhea ADR: Hyponatremia, seizures, Suicidal ideation Contraindications: Allergy, hypersensitivity Interactions: Triptans, MADI's, St John's Wort. Senitomin- norepi reuptake inhibitors (SNRIs) SNRI's: Desvenlafaxine MOA: ↑ wl of serotonin + norepi Uses: Depression, anxiety, chronic pain SE: Nausea, Sweat, loss appetite, drowsiness or insomnia, erectile dysfunction ADR: HTN or orthostatic hypotension, suicidal ideation, seizure, & NMS (rare) p271 Contraindication/interactions: Hypersensitivity, MADI, linesolide. Tricyclic antidepressants (TCAs) TCA's (Amitryptilline): original therapy 4 depression, long ago. Fallen out of favor, HIGH mortality in OD. MOA: Similar to SNRI's, ↑ Serotonin + norepi Uses: Depression, bed wetting, OCD, fibromyalgia, neuropathic pain. SE: Sedation, postural hypotension, urinary retention, dry mouth, wt gain ADR: Seizures, respiratory depression, arrhythmias Precautions: Use of caution in pt with hx of cardiovascular disease. Interactions: MAOIs, CNS depressants MAO Inhibitors (MAOIs) MAOIs (Phenelzine): RARELY used 4 depression due to risk 4 HTN crisis when taken with tyramine rich foods + certain meds. MOA: MAO degrades serotonin, norepi, dopamine MADI: Prevent breakdown of neurotransmitters
