Physician Assistant Program Application Checklist Prerequisites for Application: ☐ Bachelor Degree • Must be conferred by the application deadline. ☐ Grade Point Average • Minimum CASPA calculated overall science GPA of 3.0. ☐ Required coursework – see our website for more information on course requirements. All courses must be completed with final grades available by the application deadline. • General Biology or Zoology with lab, one semester • General Chemistry with lab, one semester • Organic Chemistry with lab, one semester • Human or Animal Physiology, one semester • Biochemistry, one semester • Three upper level biology courses (300-400 level), one semester each. See website for suggested courses. ☐ Technical Standards – see our website for more information on each standard. • Observation • Communication • Motor • Intellectual-conceptual, integrative, and quantitative abilities • Behavior and social attributes Required Documents for Submission: ☐ Completed CASPA application • Must be in verified status by the application deadline. ☐ Computer-Based Assessment for Sampling Personal Characteristics Test (CASPer) • The minimum required score is the 40th percentile. • See the CASPer website (https://takealtus.com/dates-times/) for available test dates. ☐ Three letters of recommendation • One letter from a MD or PA • One letter from a Professor or Academic Advisor – must be able to comment on your academic performance • One letter from an employer or supervisor ☐ Supplemental application • Once we receive your verified CASPA application, the program will email instructions to access and complete the required supplemental. ☐ Application fee • $45 supplemental application fee in non-refundable. • Paying the fee does not automatically submit your application. You must submit your application after completing all components. ☐ International Students Only • Foreign transcripts must be verified by World Education Services (WES). • TOEFL scores are required for applicants who did not complete four years of undergraduate education the US, Canada, or UK.
