ACTIVE LEARNING TEMPLATE: Medication PURPOSE OF MEDICATION Expected Pharmacological Action Therapeutic Use Lowers BP by speciﬁc inhibition of the angiotensin-converting enzyme (ACE). This interrupts conversion sequences initiated by renin that forms angiotension II, a potent vasoconstrictor Complications Indomethacin and other NSAIDS cam cause antihypersensitivity, potassium sparing diuretics may cause hyperkalemia, may increase lithium levels, Contraindications/Precautions Hx of angiodema related to treatments with ACE inhibitors, sensitivity to ACE inhibitors, hypotension. Improves cardiac output and exercise tolerance. Aldosterone is also reduced,thus permitting a potassium-sparing effect. Migraine prophylaxis. Medication Administration Hypertension: Adult: PO 10 mg once/day, may increase up to 20-40 mg 1-2 x a day. Heart Failure: Adult: PO 2.5 mg daily may increase to higher tolerated doses. Acute Mi: PO 2.5 mg, then 5 mg after 24 hr.,10 mg after 48 hr., then 10 mg once daily. Nursing Interventions Measure BP prior to dosing. Monitor closely for signs of angioedema in extremities, lips, face, tongue, ect. Interactions Dizziness, fatigue, hypotension, chest pain, nausea, hyperkalemia, blurred vision, increased BUN, dyspnea, cough. Monitor serum potassium and sodium levels to avoid hyponatremia and hyperkalemia. Client Education Evaluation of Medication Effectiveness Pt. came in for observation due to elevated blood pressure. Took patients blood pressure during the assessment and noted a BP of 187/98. Administered 10 mg of Lisinopril orally to the pt. Reassessed the patient 90 minutes later, Be aware of importance of a proper diet, including sodium and potassium restrictions. DO NOT use salt substitute containing potassium. Notify prescriber if any indication of infection (sore throat, fever) arises. Discontinue drug if an allergic reaction arise (swollen face, mouth, feet and hands, hoarseness,) or sudden breathing problem arises
