Antithrombin Deficiency: AT is a protein that inhibits thrombin and certain coagulation factors. AT can also play a role in reducing inflammation within the endothelium of blood vessels. AT can be acquired by 4 mechanisms: decelerated consumption of AT (ex: DVT) decreased synthesis of AT (ex: hepatic dysfunction) excretion of AT (ex: nephrotic syndrome) medication induced (ex: estrogens) most often inherited and can lead to venous thrombosis (especially when AT levels <60%) most common sites of thrombosis: legs and within the venous system Pts w/ AT deficiency can have heparin resistance and can require ↑ doses of heparin to get adequate anticoagulation Pts w/ AT deficiency should encourage their family members to be tested for the condition (because remember it's inherited)
