Treatment for COPD Meds Zosyn (piperacillin/tazobactam) – anti-infective, used to treat skin and skin structure infections Tylenol (acetaminophen) – antipyretics, NSAID, used to relieve mild pain and fever Diagnostics: Blood tests Wound culture Ultrasound Concept Map Template Lab work: Hemoglobin 7.5, WBC 16, lymphocytes 64 Signs and SymptomsSubjective: Patient states pain is 7/10, pain located in lower extremities, she states that the right leg is worse than in the left, 53 years old Objective: Female, obese, both lower extremities are edematous, warm and firm skin on lower extremities, skin on right leg is dry and flaky NSG DX Cellulitis Patho: Cellulitis – a common, potentially serious bacterial skin infection. The affected skin is swollen and inflamed and typically painful and warm to the touch. Cellulitis usually affects the lower legs, and infection occurs when a break in the skin allows bacteria to enter (Mayo Clinic, 2022) *Acute pain related to inflammatory changes in tissues as evidenced by edema and pain rating of 7/10. – Risk for situational low self esteem related to disturbed body image. – Deficient knowledge related to unfamiliarity with condition as evidenced by worsening of condition. Patient Goal: Patients pain will be reduced to a 4/10 at the affected site. Troy BSN Student will perform a detailed skin assessment, including the affected site, at least once every shift. Troy BSN Student will keep the skin clean and dry, washing the affected area once each shift. Troy BSN Student administer pain medication and antibiotics as ordered. Troy BSN will reposition the patient q2h to prevent further skin breakdown due to the patient’s immobility. Troy BSN will elevate the lower extremities to alleviate swelling.
