NURSING CARE PLAN NURSING DIAGNOSIS# Fatigue related to increased energy requirements as evidenced by verbalizations of lethargy. ASSESSMENT Patient verbalizes that she is feeling fatigued and having trouble effectively pushing during contractions. EXPECTED OUTCOMES Patient will appear relaxed in between contractions, saving her energy for the next contraction. Patient will verbalize a sense of control over labor process. INTERVENTIONS 1-Assess degree of fatigue. 2-Encourage patient to close eyes and relax in between contractions. 3-Provide encouragement for the efforts that the patient makes in pushing during contractions. 4-Dim the lights. 5-Keep patient informed to the status of labor and estimated remaining length. SCIENTIFIC RATIONALE EVALUATION 1-Understanding the patient’s level of fatigue will help in knowing how large her remaining energy reserves are. 2-A comfortable position and a break between contractions will temporarily refresh the patient’s energy. 3- Reminding the patient that labor is progressing may allow her to find more energy for pushing and progressing labor further. 4-Dimming the lights will provide a more relaxing environment. 5-Letting the patient know that labor is progressing toward the goal of delivery will allow the patient to realize that her efforts are fruitful. Patient tried to relax during her contractions, but had a hard time relaxing because she could not catch her breath. With the help of O2, patient appears more relaxed and has a sense of control of the laboring process.
