‭Parkinson's Disease and Other Parkinsonian Syndromes: Understanding‬ ‭Movement Disorders‬ ‭-‬‭Parkinson disease : Discretes at the beginning of evolution.‬ ‭o‬ ‭Small steps, delay in initiation, kinetic jams, festination.‬ ‭o‬ ‭Postural instability.‬ ‭o‬ ‭Motor complications: fluctuations and involuntary movements‬ ‭(dyskinesias: dystonia, chorea).‬ ‭o‬ ‭On-Off phenomena.‬ ‭o‬ ‭Disappearance of the automatic swinging of the upper limbs when walking.‬ ‭o‬ ‭Freezing: walks slowly, then suddenly stops and falls forward. Festination: sick‬ ‭o‬ ‭person who chases his sense of gravity.‬ ‭o‬ ‭Decomposition of the U-turn.‬ ‭-‬ ‭The most common cause of Parkinson's syndrome: Parkinson's disease, but‬ ‭it is not the exclusive cause (example: neuroleptics).‬ ‭-‬‭Other parkinsonian syndromes: o‬ ‭Neuroleptics.‬ ‭o‬ ‭Degenerative: PSP, AMS, DCL.‬ ‭-‬‭Lacunar syndrome: o‬ ‭Set of functional signs and clinical signs which result from the‬ ‭presence of multiple lacunar infarcts (cerebral ischemia which‬ ‭results from damage to a small perforating artery).‬ ‭o‬ ‭Patients do not walk, pseudobulbar syndrome, cognitive disorders.‬ ‭Gait disorders and involuntary movements – EQ‬ ‭-‬‭Choreic approach.‬ ‭-‬‭Dystonic walking.‬ ‭-‬‭Ticker approach.‬ ‭-‬‭Myoclonus.‬ -‭ ‬‭Orthostatic tremor.‬ ‭Painful walking‬ ‭-‬‭Arterial claudication.‬ ‭oIntermittent + pain + no hypotonia.‬ ‭-‬‭Radicular claudication: o‬ ‭Intermittent + pain + hypotonia (“flabby legs”). Single‬ ‭o‬ ‭or multi-rooted. Narrow lumbar canal.‬ ‭o‬ ‭-‬‭Painful lameness:‬ ‭osteoarticular pathology.‬ ‭-‬‭Tarsal tunnel syndrome.‬ ‭-‬‭Muscular pathology: metabolic myopathy polymyositis.‬
