CLIENT MEDICATION FORM Trade & Generic Names Ondansetron (Zofran) Classification Antiemetic Pharmacokinetics (absorption, [onset, peak, duration], distribution, metabolism, excretion) Oral ondansetron is well absorbed from the GI tract and undergoes some first-pass metabolism. The drug’s half-life is 3 to 5.5 hours in most patients and 9 to 20 hours in patients with moderate or severe liver impairment. With the oral form, action begins in 30 to 60 minutes and peaks in about 2 hours. With the IV form, onset and peak of drug action are immediate. The bioavailability is slightly increased by the presence of food but is unaffected by antacids. Client Order (include dosage, route, & frequency) 4 mg = 2mL, IV Push, q8hR, PRN Side Effects Diarrhea, headache, dizziness, constipation, fatigue, transient elevation of liver enzymes, and pain Recommended Dosage Range PO 16 mg 1 h before anesthesia, IV 4 mg given over 2–5 min, IM 4 mg IV medication administration flow rate & dilution 0.15 mg/kg IV, with the first dose (infused over 15 minutes) 30 minutes before the start of emetogenic chemotherapy and subsequent doses given 4 and 8 hours after the first dose. Uses/Indications (individualize for client) Nausea and vomiting Mechanisms of Action Blocks serotonin receptors centrally in the chemoreceptor trigger zone and peripherally vagal nerve terminals in the CNS. This action reduces nausea and vomiting by preventing serotonin release in the small intestine. Contraindications Concomitant use of apomorphine, congenital long QT syndrome, hypersensitivity to the drug or its components Drug Interactions (include IV compatibility/incompatibility) Apomorphine, serotonin 5-HT3 antagonists, antibiotics such as clarithromycin Antidote (include dosage, route, & frequency) Nursing Implications (including client teaching) Assess patient for nausea, vomiting, abdominal distention, and bowel sounds prior to and following administration. Monitor ECG in patients with hypokalemia, hypomagnesemia, HF, bradyarrhythmias, or patients taking concomitant medications that prolong the QT interval. Instruct patient to take ondansetron as directed; Advise patient to notify health care professional immediately if symptoms of irregular heart beat or involuntary movement of eyes, face, or limbs occur.
