Defines situation that place system at financial risk, determines frequency of occurrence, intervenes, and investigates event, and identifies risks and opportunities to improve care. *** culture of safety in a culture of safety, the focus is on: o o o effective systems and teamwork to accomplish the mutual goal of safe, high-quality performance. on what (or how), rather than who, is the problem. And bringing process failures and system issues to light, in order to problem solve them in a non-biased nonthreatening way. Within an environment that reflects common features of a culture of safety there is: o o o o o o Leadership commitment to safety. Open communication founded on trust. Organizational learning. A non-punitive approach to adverse event reporting and analysis. Teamwork. Shared belief in the importance of safety Studies have found that when health care facilities create a culture of safety, their staff are more willing to report adverse events. Nurses can contribute to patient safety by sharing information, showing respect, acknowledging different perspectives when encountering roadblocks, and involving patients in their care. Interdisciplinary vs. Interprofessional Teams ***Types of teams. o Interprofessional teams - collaboration between two or more health professionals to enhance patient care. o Inter-disciplinary teams - Interdisciplinary practice is an integrated approach in which members of a clinical team actively coordinate care and services across disciplines. o Uni-disciplinary/uni-professional teams - involves functioning in isolation from other disciplines; Everyone in their own silos, no coordination or communication among those professions. o Multi-disciplinary teams - a clinical group whose members each practice with an awareness of and toleration of other discipline; Communication between silos, the various professions operate in their individual silos, but there is some communication between them. o Trans-disciplinary teams – Roles are shared beyond the boundaries of the professional scope to maximize communication, reciprocal relations, and cooperation among the team members. o Inter-sectoral team- is collaboration across various ministries, government agencies and other relevant groups to address a particular issue. ***Review and discuss the concepts and components of the IPC framework. Professional Boundaries ***Understand the CRNA (2022) professional boundaries in nurse-client relationship. o o o o Guideline 1 The nurse is accountable for establishing therapeutic relationships with their clients. Guideline 2 The nurse is responsible for maintaining healthy professional boundaries, not the client. Guideline 3 The nurse is accountable for ensuring ethical nursing care and practice in compliance with the values of the Code of Ethics for Registered Nurses (CNA, 2017). Guideline 4 The nurse is responsible for establishing therapeutic relationships for specific client health-care needs and within the boundaries of sound professional judgement and professional expectations. Boundary signs are warning signals that professional boundaries are in question or have been crossed. Boundary signs warn the nurse to stop and assess the relationship with a client. These include.  frequently thinking of the client when away from work.  frequently planning other client’s care around this client’s needs.  spending free time with the client.  sharing personal information or work concerns with the client.  feeling responsible if the client’s progress is limited.  favoring one client’s care over another’s.  sharing secrets with the client.  selective reporting of client’s behaviour (negative or positive).  swapping client assignments to provide care to a particular client.  communicating in a guarded and defensive manner when questioned regarding interactions with the client.  changing dress style for work when working with the client.  receiving gifts or continued contact/communication with the client after discharge.  denying the fact that the client is a client.  acting and/or feeling possessive about the client; and  denying that you have crossed the boundary from a therapeutic to non-therapeutic relationship. ***Refer to guidelines for the nurse-client relationship from the national Interprofessional Competency Framework
