The success of skin flaps depends on understanding their vascular anatomy, blood supply, and methods of movement. Vascular Anatomy and Blood Supply 1. Direct Vascularization o Skin flaps rely on the subdermal plexus, which is a network of interconnected blood vessels within the dermis. o Flaps are designed to include one or more direct cutaneous arteries that provide a reliable blood supply. 2. Axial vs. Random Pattern Flaps o Axial Pattern Flaps: Based on a known dominant artery that supplies the flap. These flaps have a reliable blood supply and can be designed with predictable dimensions. o Random Pattern Flaps: Lack a specific axial blood vessel and rely on the random network of vessels within the subdermal plexus. They are less reliable and suitable for smaller defects. Classification of Skin Flaps Based on Blood Supply 1. Axial Flaps o Named Flaps: Named after the artery supplying them (e.g., radial forearm flap). o Musculocutaneous Flaps: Include muscle tissue along with skin, relying on the muscle's vascular supply. 2.
