Chromophobe Carcinoma Chromophobe carcinoma is a rare subtype of renal cell carcinoma (RCC), accounting for approximately 5% of all RCC cases. It arises from the intercalated cells of the renal collecting ducts and is distinct from other forms of kidney cancer due to its unique histological, genetic, and clinical features. Pathophysiology The name "chromophobe" refers to the tumor cells' characteristic appearance under the microscope, where they exhibit pale or clear cytoplasm with prominent cell membranes, and they tend to resist staining (chromophobic). Chromophobe carcinoma cells are larger and polygonal, with perinuclear halos and distinct cell borders. The tumors are often tan or light brown and may have a central scar. Chromophobe carcinoma is associated with multiple chromosomal losses, particularly involving chromosomes 1, 2, 6, 10, 13, 17, and 21, which differentiates it from clear cell RCC, which is more commonly associated with mutations in the VHL gene. Despite these chromosomal losses, chromophobe carcinoma tends to have a better prognosis compared to other RCC subtypes due to its lower likelihood of metastasis. Clinical Manifestations Patients with chromophobe carcinoma often present with non-specific symptoms, which may include:  Flank pain or discomfort  Hematuria (blood in urine)  A palpable abdominal mass  Fatigue  Unexplained weight loss In many cases, however, chromophobe carcinoma is discovered incidentally during imaging studies for other medical issues. A definitive diagnosis is made through biopsy and histological examination. Immunohistochemical staining can aid in differentiating chromophobe carcinoma from other RCC subtypes, with markers such as CK7, CD117 (c-Kit), and parvalbumin often being positive in chromophobe carcinoma. Treatment The primary treatment for chromophobe carcinoma is surgical removal of the tumor. Depending on the size and location of the tumor, this may involve a partial nephrectomy (removal of part of the kidney) or a radical nephrectomy (removal of the entire kidney). Given its typically low grade and low metastatic potential, chromophobe carcinoma may have a good prognosis following surgery, especially if the tumor is detected early. In cases where the cancer has metastasized or recurred, treatment options may include targeted therapy or immunotherapy, although chromophobe carcinoma is less responsive to these treatments compared to clear cell RCC. Active surveillance may be an option for smaller, asymptomatic tumors. Prognosis Overall, the prognosis for chromophobe carcinoma is generally favorable, with a five-year survival rate of about 80-90% for localized disease. The low metastatic potential and slower progression compared to other RCC subtypes contribute to this positive outlook. However, the prognosis can vary depending on the tumor size, stage at diagnosis, and presence of any metastases.
