Pancreatic Carcinomas Definition: Pancreatic carcinoma, commonly referred to as pancreatic cancer, is a malignant neoplasm originating from the cells of the pancreas. It is one of the most aggressive and deadly forms of cancer due to its tendency to be diagnosed at an advanced stage and its resistance to treatment. Epidemiology:  Incidence: Pancreatic cancer is the 12th most common cancer worldwide but is a leading cause of cancer-related death. It is more common in older adults, with the majority of cases occurring in individuals over 65 years of age.  Risk Factors: Key risk factors include smoking, chronic pancreatitis, family history of pancreatic cancer, genetic syndromes (e.g., BRCA2 mutation, Lynch syndrome), obesity, and type 2 diabetes. Pathophysiology:   Types of Pancreatic Cancer: o Adenocarcinoma: The most common type, accounting for approximately 90% of pancreatic cancers, originates from the ductal epithelium and is often referred to as pancreatic ductal adenocarcinoma (PDAC). o Other Types: Include acinar cell carcinoma, pancreatic neuroendocrine tumors (PNETs), and rare forms like squamous cell carcinoma. Molecular Changes: Pancreatic adenocarcinoma is associated with genetic mutations in oncogenes (e.g., KRAS), tumor suppressor genes (e.g., TP53, CDKN2A), and DNA repair genes (e.g., BRCA2). These mutations drive the aggressive nature of the disease and its resistance to conventional therapies. Clinical Presentation:  Symptoms: Pancreatic cancer often presents with nonspecific symptoms, which may include: o Jaundice: Due to obstruction of the bile duct, leading to bilirubin buildup. o Abdominal Pain: Often in the upper abdomen or back, which may be constant or intermittent. o Weight Loss: Unexplained weight loss and loss of appetite. o Digestive Issues: Nausea, vomiting, and changes in stool color or consistency (steatorrhea). o New-Onset Diabetes: Especially in older adults, due to pancreatic dysfunction affecting insulin production. Diagnosis:  Imaging Studies: o CT Scan: The primary imaging modality for diagnosis, staging, and assessing resectability. o MRI and Endoscopic Ultrasound (EUS): Useful for detailed imaging and local staging, especially to identify small lesions or lymph node involvement. o PET Scan: May be used for evaluating distant metastases.  Biopsy: A definitive diagnosis is often obtained through a biopsy, which can be performed via endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) or percutaneously.  Blood Tests: Tumor markers such as CA 19-9 can be elevated in pancreatic cancer but are not specific to the disease. They are more useful for monitoring response to treatment and disease progression. Management:   Surgical Treatment: o Whipple Procedure (Pancreaticoduodenectomy): The most common surgery for resectable pancreatic cancer, involving removal of the head of the pancreas, duodenum, gallbladder, and bile duct. o Distal Pancreatectomy: Removal of the body and tail of the pancreas, often indicated for tumors in these regions. o Total Pancreatectomy: Removal of the entire pancreas, considered in specific cases. Chemotherapy: Often used adjuvantly after surgery or as primary treatment for unresectable tumors. Common regimens include FOLFIRINOX or gemcitabine-based combinations.  Radiation Therapy: Can be used in combination with chemotherapy for locally advanced cases or to alleviate symptoms.  Targeted Therapy and Immunotherapy: Research is ongoing to develop therapies targeting specific genetic mutations and immune checkpoint inhibitors. Prognosis:  Survival Rates: The overall prognosis for pancreatic cancer is poor, with a 5-year survival rate of about 11%. Most patients present with advanced disease, where treatment focuses on palliative care to manage symptoms and improve quality of life.  Follow-Up: Regular monitoring through imaging and tumor markers is essential for detecting recurrences and managing complications.
