Pathophysiology: The exact cause of NEC remains unclear, but it is believed to involve a combination of factors including immaturity of the gastrointestinal tract, abnormal bacterial colonization, intestinal ischemia, and an immature immune response. The initial insult often leads to mucosal injury, which allows bacteria to invade the intestinal wall, triggering an inflammatory cascade and subsequent tissue necrosis. Clinical Presentation: NEC typically presents with nonspecific symptoms initially, such as feeding intolerance, abdominal distension, and lethargy. As the disease progresses, infants may develop bilious vomiting, bloody stools, temperature instability, and signs of sepsis. Advanced stages can lead to peritonitis, bowel perforation, and septic shock, necessitating urgent surgical intervention. Diagnosis: Diagnosis of NEC is based on clinical suspicion and supported by radiological findings. Abdominal X-rays may show pneumatosis intestinalis (air within the bowel wall), portal venous gas, or free intraperitoneal air, indicative of bowel wall necrosis and perforation. Blood tests often reveal metabolic acidosis, thrombocytopenia, and leukopenia. Management: Management of NEC involves a multidisciplinary approach including neonatologists, pediatric surgeons, and nutritionists. Initial treatment focuses on bowel rest, intravenous fluids, and broad-spectrum antibiotics targeting gut flora. In severe cases or with evidence of perforation, surgical intervention such as bowel resection or ostomy creation may be necessary. Prognosis and Complications: The prognosis of NEC varies depending on the severity of the disease and gestational age of the infant. Complications can include short bowel syndrome, neurodevelopmental delays, and long-term feeding difficulties. Survivors may require ongoing multidisciplinary care to address these issues.
