Irritable Bowel Syndrome PATHOPHYSIOLOGY - not well understood Visceral Hypersensitivity → food ingestion → osmotically pull H₂O in bowel → H₂O stays in bowel wall ↑ carbohydrates 1. Central Sensitization 2. More Stretch of bowel wall send stretch signals to brain (CNS) ↑ amplification of signals ↑ abdo pain (cramping & bloating) Hypermotility - bowel spasms unduly → more excessively → makes pain worse eg. depression, anxiety → may be related to underlying psychiatric disorders ↑ in contractile spasms SK (Gut-Reflex) → colonic hypermotility Enteric plexus reflexive movement move things in bowel very quickly won't have enough time for proper absorption bloating → my common IBS-D: diarrhoea IBS-C: constipation IBS-M: mixture of diarrhoea & constipation altered bowel habits + abdo pain altered bowel habits + abdo pain IBS-M: mixture of diarrhoea & constipation
