ACTIVE LEARNING TEMPLATE: Diagnostic Procedure ECG PROCEDURE NAME ____________________________________________________________________ REVIEW MODULE CHAPTER ___________ Description of Procedure An electrocardiogram (ECG or EKG) records the electrical signal from the heart to check for different heart conditions. Electrodes are placed on the client's chest to record the heart's electrical signals, which cause the heart to beat. Indications -Arrhythmias -Coronary Artery Disease -Previous Heart Attack -To see how well certain heart disease treatments, such as a pacemaker, are working -Family hx of heart disease Interpretation of Findings -Identifies arrhythmias and various kinds of heart rhythms e.g. tachycardia, bradycardia -Can show evidence of a previous heart attack or one that's in progress -An ECG can provide clues about enlargement of the chambers or walls of the heart, heart defects and other heart problems. Potential Complications -ECG may have an unclear or failed reading due to shivering, body movement, the electrodes coming off, or an interference with a pacemaker. -The electrodes may not be able to stick. CONSIDERATIONS Nursing Interventions (pre, intra, post) -Pre: Help the patient change into their hospital gown. If needed, shave the areas on the places where the elctrodes will stick -Intra: Attach the elctrodes on the patient's body as directed (on the chest and the limbs) -Post: Help patient change back into their normal clothes if needed, otherwise patient can resume normal activities unless directed otherwise by HCP Client Education No special preparations are necessary for a standard electrocardiogram. Tell the client to inform the doctor about any medications and supplements they take. These can often affect the results of the test. Nursing Interventions -Instruct the patient to lie still when placing the electrodes so the ECG can have a proper reading -If electrodes are not sticking, shave the areas with hair or use an alcohol wipe for better adhesion
