Chlorpromazine Therapeutic class: 1st gen antipsychotic ; schizo drug Pharmacologic class: D2 dopmine receptor antagonist, phenothiazine Actions and uses : provides symptomatic relief of positive s/s of schizo and controls manic s/s in pts w schizoaffective d/o. Must take 7-8 wks before improvement is noted Can use IM or IV injections for extreme agitation- act within minutes Can also control severe nausea and vomiting Peak effects take 6 wks- months to show ADMIN ALERTS: Cannot be crushed or opened if in sustained release forms When given IM only use upper outer quadrant of the buttocks; should remain upright 30-60 mins after injection and rise slowly after. DC gradually to avoid n/v, dizziness, tremors, dyskinesia IV forms should only be used during sx or for severe hiccups PREGNANCY CATEGORY C Onset : 30-60 mins Peak: 2-4hrs PO 15-20min IV/IM Duration: 30hrs ADVERSE EFFECTS: dizziness, drowsiness, and orthostatic hypotension. EPS (mostly in older females and ped patients that are dehydrated) , neuroleptic malignant syndrome. BLACK BOX WARNING: older pts w dementia related psychosis are at an increased r/o death when taking conventional antipsychotics. CONTRAINDICATIONS: don’t use during alcohol withdrawal or when the pt is comatose. INTERACTIONS: interacts w several drugs (sedatives, TCAS , seizure meds) → taking w TCAS increases BP, taking w antiseizure meds lowers the seizure threshold. TX of overdose: no specific tx for overdose. Treat symptoms. EPS can be tx w antiparkinson meds, barbiturates, anticholinergics, or Benadryl ( avoid and monitor RR depression w these txt’s)
