Estradiol and Norethindrone (Ortho-Novum) Therapeutic class: combination oral contraception Pharmacologic class: estrogen-progestin Actions/uses : to prevent pregnancy and indication for which it is nearly 100% effective. When an appropriate combo of estrogen and progestin is present in the bloodstream, the release of FSH and LH is inhibited, thus preventing ovulation. Off label indications for the drug → acne vulgaris, endometriosis, hypermenorrhea, dysfunctional uterine bleeding ADMIN ALERTS: tabs must be taken exactly as directed If the dose is missed, take as soon as you remember or take 2 tabs the next day. PREGNANCY CATEGORY X Onset: 30-60 mins (1 month for contraception) Peak: 1 month Duration: up to 27hrs A/E: most frequent are: nausea, breast tenderness, weight gain, breakthrough bleeding. Less common effects: edema, changes in vision, gallbladder disease, nausea, abd cramps, changes in urinary function, dysmenorrhea, breast fullness, fatigue, skin rash, acne, HA, vaginal candidiasis, photosensitivity and changes in urinary patterns. Cardiovascular effects (most serious) – include HTN, thromboembolic d/o BLACK BOX WARNING: cigarette smoking increases the r/o serious cardiovascular a/e in women who are taking hormonal contraceptives containing estrogen. This risk increases w age >35 yrs old and w heavy smoking >15 cigarettes/day. Contraindications: hormonal contraceptives are contraindicated in women w the following conditions Current or past hx of thromboembolic d/o , stroe, or CAD, live tumors, known or suspected carcinomas of breast, endometrim or other estrogen dependent tumors; abnormal bleeding INTERACTIONS: should not be used w tranexamic acid d/t increased risk for thromboembolic events. Rifampin, some abx, barbiturates, anticonvulsants, protease inhibitos, and antifungals decreased the efficacy of HCS , increasing the risk or breakthrough bleeding and the possibility of pregnancy Ortho-novum can decrease the effects of warfarin, heparin, and other anticoagulants→ leading to thromboembolic events Tx for overdose: NONE
