5 yo boy brought to ED by his parents with 3 days of cough and runny nose and increased WOB since last night. He has had previous episodes of wheeze which was treated with Ventolin in the past. This is his fourth episode within the last 4 months. On examination, his observations are: RR-50/min, PR- 160/min, SPO2-95% in air. His weight is 18 kg. he is talking in phrases. There is a moderate increase in WOB, and bilateral wheeze with good air entry. What is the most likely diagnosis of this child? How will you initially treat him in ED? Demonstrate to the parents, how to use a puffer and spacer Give 4 criteria for discharge You have treated him effectively in ED and now he is ready to be discharged. Write a weaning plan. Write an action plan. Answers: Moderate asthma exacerbation triggered by an URTI infection (most likely viral) Would treat him as a moderate asthma: 6 puffs of Ventolin via MDI via a spacer as three doses every 20 minutes Then I would re-assess after 30 minutes of third dose If they are not improving, I would give them ipratropium bromide as four puffs as three doses every 20 minutes Then I would re-assess if still not improving I would treat as a severe asthma Take off the cap -> s hake the puffer well before use -> insert into spacer -> place the mask over the child’s mouth and nose and press one puff. Take out and shake again before each puff. Clean it with warm water and soap and then leave to air dry. Clean it weekly. Or for older children seal the lips tightly around the lips. Do not need a reliver more than once every three hours. No features of respiratory distress – including grunting, recessions, increased work of breathing, increased resp rate. Tachypnoea or SOB Do not require oxygenation -> oxygen saturation of more than 94% at room air. Able to tolerate oral and fluid intake Parents have been given an asthma plan and have been educated 5yo boy presenting with moderate acute asthma. Acute management? Ventolin weaning plan? Asthma action plan Educate parents how to use the MDI with spacer and mask What if he has had 5 admissions to the ward in 5 months and one to ICU? Answer for e Refer to the paediatric respiratory specialist Consider a preventor therapy. Arrange asthma education plan with escalation steps Arrange written asthma management plan with escalation steps. Screen for adherence, technique, triggers and psychosocial stressors.
