Disruptive Mood Dysregulation Disorder Definition Extreme irritability and severe recurrent outbursts of anger - Verbal or behavioural Epidemiology • • • More women than men Most commonly onset at less than 10. ODD and depressive disorders are common comorbidities Aetiology • • • • Psychological trauma/or abuse e Malnutrition Vitamin deficiencies Neurological problems e.g. migraines Diagnosis DSM-V • • • • Severe outburst of anger (verbal or behavioural) lasting for at least 12 months (without interruption for at least three consecutive months) on average at least three times a week that are grossly out of proportion in intensity or duration to the situation and the child’s developmental level. Persistent anger or irritability in between outbursts which is observable by others. Trouble functioning due to irritability in various situations e.g. at home, school, with peers. The patient has to be between 6 to 18 years of age for this diagnosis to be considered. Management • • • CBT - Individual and family psychotherapy Parent management training Pharmacotherapy to address irritability and mood problems - Stimulants - Anti-depressants - Atypical anti-psychotics Prognosis At a higher risk of developing major depressive disorder and anxiety disorders in adulthood.
