SOAPIER concept: S Subjective data:This note deals with the problem from the point of viewpatient. The patient's expressions regarding his concerns and complaints were recorded as direct quotes from the summary relating to the diagnosis. For people who are mute, the data section behind S is marked with "Zero" or "X", this indicates that the person is mute. In babies or young children, this subjective data can be obtained from their parents. Subjective data confirms the diagnosis that will be made. O Objective Data:These data provide evidence of the patient's clinical symptoms and facts related to diagnosis. Physiological data, honest observation results, information on technological studies (laboratory results, X-rays, CTG recordings, ultrasound, etc.) can be classified in this category. There are opinions that include reports from families also in this category. What the midwife or midwife can observe will be a significant component of the diagnosis that will be made. A Analysis / Assessment = study:problem or diagnosis madebased on subjective and objective data / information collected and concluded. Because the patient's condition continues to change and there is always new information, both subjective and objective, and is often expressed separately, the assessment process is a dynamic process. Frequent analysis is important in following the patient's progress and ensuring that new changes are quickly noticed and can be followed so that appropriate action can be taken.
