Premature Rupture of the Membrane Initial Assessment and Management HISTORY When did the fluid loss happen? How much fluid is there? What is the colour of the fluid? What is the consistency of the fluid? Does the fluid have an odour? Is there blood? Is there meconium? How much fluid is there? Are there any contractions? Associated features Fever Tachycardia Changes to rate of breathing GBS status Obstetric history Has this happened before? What pregnancy number is this? Is this a planned or an unplanned pregnancy? Was this spontaneous or assisted? Any complications with this pregnancy Previous pregnancies Mode of delivery for the previous pregnancies GYN history Menstrual cycle Regularity, duration, colour, pain, clots, amount and age of menarche Past med history Medications Past surg history Family history Management when there is yellow-green liquor This means that there is meconium passage This raises concern for: Risk of infection -> chorioamnionitis Fetal stress or hypoxia Meconium aspiration syndrome Continuous CTG monitoring If appropriate -> induction of labour after confirmation or ruptured forewaters Give mother IV penicillin to mother After delivery of the head -> suction the airways/trachea Call paediatrics Features of chorioamnionitis Fever Uterine/abdominal tenderness Maternal leucocytosis Maternal tachycardia Maternal fever Fetal tachycardia on CTG Raised CRP
