Kidney Failure Describe the epidemiology of renal failure. Outline Tx options for renal failure (conservative, dialysis, transplant). AKI definition as per KDIGO guidelines ↑ in Scr by ≥0.3 mg/dL within 48 hrs (≥26.5 μmol/L) ↑ in Scr to ≥1.5x baseline which is known or presumed to have occurred within the prior 7 days Urine vol <0.5 mL/kg for 6 hrs Fluid resuscitation to correct pre-renal factors Monitoring of UO, fluid balance chart, daily weight to assess volume status Decrease non-essential meds (e.g., ARBs, NSAIDs, etc.) Dose adjustments as per changing renal f(x) CKD complications Fluid overload → hypertension, CHF, platelet dysfunction Hypokalemia → arrhythmias, muscle weakness Hypercalcemia → nephrolithiasis Metabolic acidosis → anemia, CCF ↑ bone turnover disease → osteomalacia, osteoporosis ↑ phosphate retention → hyperphosphatemia ↑ PTH Osteomacia due to aluminum toxicity (now rare) Dialysis-related amyloidosis Uremic encephalopathy (syndrome) Uremic pruritis
