Male Catheterisation Indications • • • • • • • • • • Perioperative use in selected surgeries. Acute urinary retention of obstruction. In palliative care. Measurement of urinary output for critically unwell patient or determine urinary output following any major surgery. When strict immobilisation for trauma and surgery. Irrigate the ladder with a 3-way catheter following prostate surgery. To allow urodynamic studies to be performed to confirm bladder function. Determine residual urine volumes. Clinical scenarios where the bladder control is lost. Temporary or permanent loss. - Spinal cord injury, unconscious patients, those under spinal anaesthetics. To introduce cytotoxic or immunotherapy drugs in the treatment of carcinoma of the bladder. Contradictions • • • Renal patients on Haemodialysis or Continuous Ambulatory Peritoneal Dialysis (CAPD) must only have urinary catheterisation after discussion with the renal physician. Do not insert urethral catheter in trauma patients if there is any blood around the urethral meatus. Refer to Senior Medical Officer/Surgeon for further investigation and insertion of catheter if required. Patients who are going to be admitted to an Intensive Care Unit and require a urinary catheter post severe trauma must be catheterised following the protocol for continuous intraabdominal monitoring. Types of catheters. • • • For transurethral resection of the prostate, a 30ml or larger catheter balloon is used with a 3way catheter to aid in anchoring the catheter on the bladder neck thereby preventing the catheter from slipping into the resected prostatic cavity. Use all silicone 16–20 fg male length catheter with 10ml balloon capacity for long term catheterised patients (unless specified otherwise by Medical Officer).
